研究目的：

慢性非细菌性下颌骨骨髓炎是一种罕见的自身炎症性骨疾病，其诊断和治疗尚无标准化方案，本研究通过对基线特征、随访情况与用药方案的总结，提出一个多维度的疗效评价体系，分析预后与相关特征的可能相关性，期望减少患者由于诊疗延误导致的身心伤害，为此后的诊治提供参考。  

Objectives of Study：

Chronic nonbacterial osteomyelitis is a rare autoinflammatory bone disease for which there are currently no standardized diagnostic or treatment protocols. By summarizing baseline characteristics, follow-up data, and treatment regimens, this study proposes a multidimensional efficacy evaluation system and analyzes potential associations between prognosis and relevant characteristics. The aim is to reduce the physical and psychological harm patients suffer due to delays in diagnosis and treatment, and to provide a reference for future clinical management.

纳入标准：

1. 2020年11月至2026年3月期间于上海第九人民医院口腔外科就诊，有下颌骨反复肿胀、疼痛、张口受限等症状，且应用抗菌药物不能完全缓解，伴或不伴全身其他骨疼痛、皮肤病损、发热的患者； 2. 颌面部CT、MR：颌骨硬化/溶骨性病变/骨膜反应/骨质增生/骨膨胀，边界不清； 3. 实验室检查：白细胞计数正常者，可有促炎细胞因子、红细胞沉降率、C反应蛋白等升高； 4. 活检病理显示慢性骨髓炎，细菌培养无明显致病菌； 5. 随访时间>=1.5年。

Inclusion criteria

1.Patients who were treated at the Department of Oral and Maxillofacial Surgery at Shanghai Ninth People’s Hospital between November 2020 and March 2026 and presented with symptoms such as recurrent swelling and pain in the mandible and limited mouth opening, which did not fully resolve with antibiotic therapy, with or without other systemic symptoms such as bone pain, skin lesions, or fever; 2.Maxillofacial CT and MRI: Sclerosis/osteolytic lesions/periosteal reaction/osteophytes/bone expansion, with indistinct borders; 3.Laboratory tests: In patients with normal white blood cell counts, levels of pro-inflammatory cytokines, erythrocyte sedimentation rate, and C-reactive protein may be elevated; 4.The biopsy pathology revealed chronic osteomyelitis; bacterial cultures did not identify any significant pathogens; 5.Follow-up period of >=1.5 years;

排除标准：

1.有放化疗治疗史的，有抗骨吸收药物或抗血管生成药物治疗史的患者；
2.有确证性证据表明存在相互排斥类似疾病的；
3.单独抗菌治疗后临床和实验室表现完全且持续缓解的；
4.CT显示有明显牙源性感染灶的；

Exclusion criteria：

1.Patients with a history of chemotherapy or radiation therapy, or a history of treatment with antiresorptive or anti-angiogenic agents;
2.There is conclusive evidence indicating the existence of mutually exclusive conditions with similar symptoms;
3.Patients who achieved complete and sustained clinical and laboratory remission following antimicrobial therapy alone;
4.CT scans showing clear evidence of a dental infection focus;

Define ROIs on the 0.625 mm CT images. For each lesion, place a 3-mm-diameter ROI at the site of maximum density, away from the cortical bone, and record the mean HU value as the hyperostosis HU. Place an ROI of the same size at the site of lowest density to obtain the osteolytic area.
