研究目的：

通过半结构化访谈及文献回顾，深入了解全膝关节置换术患者在出院准备过程中的真实体验、障碍及需求，并结合德尔菲专家函询，构建基于导航护理的全膝关节置换术患者出院准备度提升方案，并初步验证基于导航护理的出院准备度提升方案在TKA患者中的应用效果。  

Objectives of Study：

(1) To explore the experiences, barriers, and needs of patients undergoing total knee arthroplasty (TKA) during the discharge preparation process through semi-structured interviews and a literature review, and to construct a navigation nursing-based program to improve readiness for hospital discharge using the Delphi method.(2) To preliminarily assess the effectiveness of the navigation nursing-based program for improving readiness for hospital discharge in patients undergoing TKA.

纳入标准：

1.①初次行择期全膝关节置换术；②年龄≥18岁；③具备基本阅读、理解和沟通能力；④知情同意且自愿参与本研究者。

Inclusion criteria

1.Patients undergoing primary elective total knee arthroplasty (TKA);
2.Aged 18 years or older;
3.Able to read, understand, and communicate effectively;
4.Provided informed consent and voluntarily agreed to participate in the study.

排除标准：

1. 出院后前往康复医疗机构进行康复者； 2. 住院期间出现严重并发症或病情变化，导致无法继续完成干预或评估者； 3. 研究过程中主动退出或资料不完整者。

Exclusion criteria：

1.Patients who were transferred to rehabilitation healthcare facilities for postoperative rehabilitation after discharge;
2.Patients who developed severe complications or experienced significant changes in their medical condition during hospitalization, resulting in an inability to complete the intervention or evaluation;
3.Patients who voluntarily withdrew from the study or had incomplete data during the study period.
