本研究旨在分析多次肠造口闭合失败儿童的管理策略与结局。  

Objectives of Study：

This study aimed to analyze the management strategies and clinical outcomes for this complex patient population.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.年龄＜14 周岁的儿童； 2.因先天性或获得性胃肠道疾病行肠造口术（回肠造口或结肠造口），且既往至少经历过1 次造口闭合术失败； 3.造口闭合术失败的定义：既往造口闭合术后，出现明确的与手术失败相关的特异性症状，具体包括：持续性或进行性加重的腹胀；反复呕吐（呕吐物含胆汁或粪汁）；反复肠炎发作；顽固性便秘（定义为经保守治疗无效，每周排便≤2 次）。

Inclusion criteria

1. Children younger than 14 years; 2.History of enterostomy (ileostomy or colostomy) for congenital or acquired gastrointestinal disease, followed by at least one prior unsuccessful attempt at closure; 3. Definition of Closure Failure: Documented onset of definitive symptoms related to surgical failure after the previous closure procedure, specifically including: persistent or progressively worsening abdominal distension;recurrent vomiting (bilious or feculent emesis);recurrent enteritis;refractory constipation (defined as <=2 bowel movements per week unresponsive to conservative management).

排除标准：

1.因肠道恶性肿瘤、活动性感染性肠炎（如肠结核）或炎症性肠病（如克罗恩病），首次行肠造口术及造口闭合术的患者； 2.法定监护人拒绝本次确定性手术，或拒绝参与本研究及术后随访的患者； 3.临床病历资料不完整，无法明确判断造口闭合术失败原因或评估预后的患者； 4.合并严重先天性或终末期心、肝、肾、神经系统疾病，无法耐受再次手术及麻醉的患者； 5.首次造口闭合术后，因外伤、医源性损伤等外界因素需再次手术的患者。

Exclusion criteria：

1. Patients whose initial enterostomy and closure were performed for intestinal malignancy, active infectious enteritis (e.g., tuberculosis), or inflammatory bowel disease (e.g., Crohn's disease); 2. Patients whose legal guardians declined the current definitive surgery or refused to participate in the study and follow-up; 3.Patients with incomplete clinical records that precluded definitive determination of the cause of closure failure or assessment of outcomes; 4. Patients with severe congenital or end-stage disease of the heart, liver, kidneys, or nervous system, rendering them unfit for reoperation or anesthesia; 5. Patients who required reoperation due to external factors such as trauma or iatrogenic injury following the initial closure attempt.
