研究目的：

动态监测机械通气危重症患者ICU住院期间股四头肌厚度变化，构建股四头肌厚度变化轨迹，探讨不同肌肉衰减轨迹与临床结局之间的关系。  

Objectives of Study：

To dynamically monitor changes in quadriceps muscle thickness during the ICU stay of mechanically ventilated critically ill patients, to identify distinct trajectories of quadriceps muscle thickness over time, and to investigate the associations between different muscle loss trajectories and clinical outcomes.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.年龄≥18岁； 2.入住ICU后接受有创机械通气治疗； 3.预计机械通气时间≥48小时； 4.ICU住院时间预计≥5天； 5.入组后能够完成至少2次股四头肌超声测量。

Inclusion criteria

1. Aged >=18 years; 2. Admission to the ICU and receipt of invasive mechanical ventilation; 3. Expected duration of mechanical ventilation >= 48 hours; 4. Expected ICU stay >=5 days; 5. Ability to undergo at least two quadriceps muscle ultrasound assessments after enrollment.

排除标准：

1. ICU入住前存在严重神经肌肉疾病； 2. 截肢或下肢严重创伤影响超声测量； 3. 股四头肌局部感染、开放伤口或严重水肿无法完成测量； 4. 妊娠患者； 5. ICU入住24小时内死亡或放弃治疗； 6. 研究期间无法获得主要结局资料者。

Exclusion criteria：

1. Pre-existing severe neuromuscular disorders prior to ICU admission; 2. Amputation or severe lower-limb trauma that interferes with ultrasound assessment; 3. Local infection, open wounds, or severe edema of the quadriceps region preventing ultrasound measurement; 4. Pregnant patients; 5. Death or withdrawal of life-sustaining treatment within 24 hours of ICU admission; 6. Inability to obtain primary outcome data during the study period.

No intervention. Based on longitudinal changes in quadriceps muscle thickness, a group?based trajectory model (GBTM) was applied to identify distinct trajectory groups. Participants could be classified into the following groups: the stable group (minimal change in muscle thickness), the slow decline group (gradual decrease in muscle thickness), and the rapid decline group (significant decrease in muscle thickness).

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

研究数据采用标准化病例报告表（Case Report Form，CRF）进行采集，包括患者基本信息、疾病严重程度评分、股四头肌超声测量结果、治疗信息及临床结局等内容。所有数据由经过培训的研究人员录入电子数据库并进行双人核查。研究数据采用唯一编码进行去标识化处理，存储于医院授权计算机及受密码保护的电子数据库中，仅研究团队成员可访问。研究结束后数据保存不少于5年。
