研究目的：

基于MRC复杂干预框架构建成人超重/肥胖患者精准体重管理方案，实施并检验方案的有效性，从而降低BMI、减少内脏脂肪面积、提高患者自我管理能力，为该类患者的管理提供依据。  

Objectives of Study：

Develop a precision weight management program for overweight and obese adults based on the MRC complex intervention framework, implement and evaluate its effectiveness to reduce BMI, decrease visceral fat area, and enhance patients’ self-management ability, thereby providing evidence for the management of this population.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1. 符合西医诊断标准( 参考 2024版《肥胖症诊疗指南》中基于体质指数（BMI）诊断标准：BMI≥24 kg/m^2为超重, ≥28 kg/m^2为肥胖，或符合基于体型诊断标准：男性腰围≥90 cm、女性腰围≥85 cm的诊断标准)； 2. 年龄18～75周岁； 3. 同意在整个研究过程中按照方案要求按时进餐、运动及监测相关指标并记录； 4. 患者知晓病情，自愿参与； 5. 熟练使用智能手机，可操作微信小程序客户端。

Inclusion criteria

1. Meet Western medical diagnostic criteria (refer to the 2024 "Guideline for the Diagnosis and Treatment of Obesity" using BMI: BMI >= 24 kg/m^2 for overweight, >= 28 kg/m^2 for obesity, or meet body-shape–based diagnostic criteria: waist circumference >=90 cm for men, >= 85 cm for women); 2. Age 18–75 years; 3. Agree to follow the protocol throughout the study, including timely meals, exercise, monitoring relevant indicators, and recording them; 4. Patient is aware of their condition and voluntarily participates; 5. Proficient in using a smartphone and able to operate the WeChat mini-program client.

排除标准：

1.胃肠疾病伴吸收不良者； 2.处于活动性肝脏疾病期，或存在明显的肝功能损害，定义为丙氨酸氨基转移酶（ALT）≧５倍正常范围上限； 3.有不稳定的或快速进展性肾脏疾病史，或存在肾功能损害，定义为血清肌酐≧133μmol/L（1.5mg/dl，男性），或≧125μmol/L（1.41mg/dl，女性）； 4.存在精神障碍、不愿意与人交流或存在语言障碍； 5.严重的心肺功能不全、甲状腺功能减退、恶性肿瘤患者； 6.妊娠状态或正处于哺乳期的女性； 7.有躯体功能障碍，影响有氧运动及抗阻运动； 8. 合并有以下情况的患者： （1）明确诊断为糖尿病并正在接受降糖药物治疗者（包括口服或注射药物）； （2）明确诊断为高血压病并正在接受降压药物治疗者。

Exclusion criteria：

1. Individuals with gastrointestinal diseases accompanied by malabsorption; 2. Those in an active phase of liver disease or with significant hepatic impairment, defined as alanine aminotransferase (ALT) >= 5 times the upper limit of normal; 3. Individuals with unstable or rapidly progressive kidney disease, or with renal impairment defined as serum creatinine >= 133 μmol/L (1.5 mg/dL, male) or >= 125 μmol/L (1.41 mg/dL, female); 4. Those with psychiatric disorders, unwillingness to communicate with others, or speech disorders; 5. Patients with severe cardiorespiratory failure, hypothyroidism, or malignant tumors; 6. Pregnant or breastfeeding women; 7. Individuals with physical disabilities that affect aerobic or resistance exercise; 8. Patients with either of the following conditions: (1) a confirmed diagnosis of diabetes mellitus currently receiving glucose-lowering medication (oral or injectable); (2) a confirmed diagnosis of hypertension currently receiving antihypertensive medication.

Participants will receive routine nursing care for overweight and obese adults in the department, which includes: providing comprehensive health education; promoting a balanced diet; guiding patients to engage in moderate- or low-intensity physical activity and exercise; advocating for healthy lifestyle habits; and encouraging smoking cessation, alcohol restriction, and salt reduction.
