研究目的：

本研究基于已结题的精益六西格玛驱动下呼吸机相关肺炎（VAP）防控优化项目，针对临床 VAP 防控中发生率偏高、集束化措施执行不规范等痛点，系统总结项目实施过程中构建的“标准化体系 + 数据化管理 + 跨部门协同”复合型干预模式及落地经验；通过对比项目实施前后 VAP 发生率、集束化措施执行规范率等核心指标的变化，验证该干预模式的临床有效性与可行性；提炼可复制、可推广的院感防控实践方案，为同类院感防控项目提供参考依据，同时通过论文撰写固化项目成果，推动院感防控工作的科学化、标准化发展。  

Objectives of Study：

This study is based on a completed Lean Six Sigma-driven project for optimizing the prevention and control of ventilator-associated pneumonia (VAP). It addresses pain points in clinical VAP prevention, such as high incidence rates and inconsistent implementation of bundled measures. The study systematically summarizes the 'standardized system, data-driven management, and cross-department collaboration' intervention model developed during the project and the practical experience of its implementation. By comparing key indicators such as VAP incidence and the compliance rate of bundled measures before and after the project, it verifies the clinical effectiveness and feasibility of this intervention model. The study also extracts replicable and scalable hospital infection prevention practices to provide a reference for similar projects, while solidifying the project results through paper writing, promoting a more scientific and standardized development of hospital infection prevention work.

纳入标准：

① 研究时间段内于上海儿童医学中心接受呼吸机支持治疗（气管插管或气管切开）的患儿；② 临床资料完整，可通过电子病历系统、感染监测系统获取VAP感染情况、集束化措施防控情况等核心研究数据。

Inclusion criteria

1. Children who received ventilator support therapy (tracheal intubation or tracheotomy) at Shanghai Children's Medical Center during the study period; 2. With complete clinical data, core research data such as VAP infection status and prevention and control measures through bundled strategies can be obtained through the electronic medical record system and infection monitoring system.

排除标准：

① 临床资料不完整，核心研究数据缺失无法补充者；② 研究期间转院或自动出院，无法完成全程数据追溯者。

Exclusion criteria：

1. Patients with incomplete clinical data and missing core research data that cannot be supplemented; 2. Patients who transferred to other hospitals or voluntarily discharged during the study period, making it impossible to complete the full data traceability.

A database was established using a spreadsheet (Excel), where core data such as ventilator usage and VAP infection were directly exported from the hospital's electronic medical record system. Data related to bundled measures were sourced from on-site observation records during the project implementation period (weekly on-site observations and monthly data summaries). The exported and summarized data were cleaned and verified to ensure accuracy and completeness before the database was locked. Sensitive data were encrypted and stored, and the database was archived in accordance with specifications, strictly adhering to hospital data security management requirements.
