构建多模式超声图像(二维灰阶超声、瞬时应变弹性成像、超声造影)甲状腺结节恶性风险模型,评价人工智能方法对C-TIRADS 3-5类结节恶性风险评价的准确度,以及人工智能模型辅助下不同年资医师对甲状腺恶性风险的诊断差异。  

Objectives of Study:

This study aims to construct a multimodal ultrasound-based malignancy risk model for thyroid nodules incorporating two-dimensional grayscale ultrasound, transient elastography, and contrast-enhanced ultrasound, and to evaluate the diagnostic accuracy of artificial intelligence in malignancy risk assessment of C-TIRADS category 3–5 nodules, as well as the diagnostic differences among physicians with different levels of experience under AI assistance.

Description for medicine or protocol of treatment in detail:

 

纳入标准:

1. 回顾性研究:收集 2022 年 1 月至 2025 年 11 月患者资料,年满 18 岁;二维灰阶超声检查发现甲状腺结节并判断为 C-TIRADS3-5;有 1 月内超声造影检查、瞬时弹性成像检查、常规灰阶超声检查;有 3 月内超声引导细针抽吸或手术,获得病理学结果。 2. 前瞻性研究:收集 2025 年 12 月至 2026 年 6 月患者资料,年满 18 岁;二维灰阶超声检查发现甲状腺结节并判断为 C-TIRADS3-5;1 月内需行超声造影检查、瞬时弹性成像检查、常规灰阶超声检查;3 月内需行超声引导细针抽吸或手术,获得病理学结果。

Inclusion criteria

1. Retrospective study: Data of patients aged 18 and above were collected from January 2022 to November 2025. Two-dimensional gray-scale ultrasound examination detected thyroid nodules and diagnosed them as C-TIRADS3-5. There are contrast-enhanced ultrasound examinations within one month, transient elastography examinations, and conventional gray-scale ultrasound examinations. There were three months of ultrasound-guided fine-needle aspiration or surgery, and pathological results were obtained. 2. Prospective study: Data of patients aged 18 and above were collected from December 2025 to June 2026. Two-dimensional gray-scale ultrasound examination detected thyroid nodules and diagnosed them as C-TIRADS3-5. Contrast-enhanced ultrasound examination, transient elastography examination and conventional gray-scale ultrasound examination should be performed within one month. Ultrasound-guided fine-needle aspiration or surgery must be performed within three months to obtain pathological results.

排除标准:

1. 二维灰阶超声、弹性成像、超声造影图像不清晰; 2. 病理或细胞学检验没有明确结果; 3. 既往有甲状腺手术史。

Exclusion criteria:

1. Poor image quality on two-dimensional grayscale ultrasound, elastography, or contrast-enhanced ultrasound; 2. Inconclusive histopathological or cytological results; 3. Prior history of thyroid surgery.
