1. 技术与产品研发目标 临床验证:验证“无接骨板内固定新技术”治疗踝关节骨折的近期总体功能(主要终点)非劣效于金标准“接骨板内固定技术”,同时评估并证实本系统在促进骨折快速愈合、降低术后疼痛、减少软组织并发症及内固定物二次取出率等次要终点方面的显著优效性与独特优势。 产品开发与迭代:依托本项目的临床研究与生物力学验证,完成“微创动态稳定应力刺激系统”核心组件的研发、优化与转化。具体包括开发应力响应式骨块钉以及温度响应式形状记忆合金促骨愈合钉和二次加压钉。 2. 智能评估与康复指导目标 依托RCT高质量的临床与影像数据,开发并验证一套基于人工智能深度学习的CT影像骨折愈合智能评估系统。提供客观、精准、可量化的骨折愈合评价新方法,为患者提供个性化的精准康复方案,填补“精准康复”领域的空白。 |

Objectives of Study: |
This project aims to clinically validate that the novel plate-free internal fixation technique for ankle fractures is non-inferior to the current gold-standard plate fixation technique in terms of short-term overall functional outcomes, while demonstrating significant superiority and distinctive advantages in accelerating fracture healing, reducing postoperative pain, minimizing soft-tissue complications, and lowering the rate of secondary implant removal. Based on the clinical investigation and biomechanical validation, the project will further advance the development, optimization, and translational application of the core components of a minimally invasive dynamic stabilization stress-stimulation system, including stress-responsive fracture fragment screws, temperature-responsive shape-memory alloy osteogenic healing screws, and secondary compression screws. In addition, by leveraging high-quality clinical and imaging data derived from a randomized controlled trial (RCT), the project seeks to develop and validate an artificial intelligence deep learning-based CT imaging system for intelligent assessment of fracture healing, thereby providing an objective, accurate, and quantifiable method for evaluating bone healing and enabling personalized, precision rehabilitation strategies for patients. |

1. 年龄>18岁的成年患者; 2. 踝关节骨折(受伤至手术时间<=3周); 3. 骨折分型符合以下任一类型:单纯外踝骨折、双踝骨折、三踝骨折; 4. 骨折存在移位,经评估具有明确的手术指征; 5. 患者理解本研究并自愿签署书面知情同意书。 |

Inclusion criteria |
1. Adult patients over 18 years old; 2. Ankle fracture (time from injury to surgery <= 3 weeks); 3. Fracture type belongs to any of the following: isolated lateral malleolus fracture, bimalleolar fracture, or trimalleolar fracture; 4. The fracture is displaced and has been evaluated to have a clear indication for surgery; 5. The patient understands the study and voluntarily signs a written informed consent form. |

排除标准: |
1. 开放性骨折(Gustilo-Anderson III型); 2. Pilon骨折(累及胫骨远端关节面的粉碎性骨折); 3. 伴有需紧急探查处理的神经或主要血管损伤; 4. 病理性骨折或严重骨质疏松(DEXA扫描T值<-3.0); 5. 合并严重糖尿病(糖化血红蛋白HbA1c>8.5%)且伴有周围神经病变或血管闭塞性病变; 6. 存在严重全身性疾病,无法耐受麻醉或手术(ASA分级 > III级); 7. 孕期或哺乳期妇女; 8. 存在精神障碍、认知障碍或预期术后依从性差,无法配合随访者。 |

Exclusion criteria: |
1. Open fractures (Gustilo-Anderson type III); 2. Pilon fractures, defined as comminuted fractures involving the distal tibial articular surface; 3. Fractures associated with nerve injury or major vascular injury requiring emergency exploration and management; 4. Pathological fractures or severe osteoporosis (DEXA T-score < -3.0); 5. Severe diabetes mellitus (glycated hemoglobin HbA1c > 8.5%) accompanied by peripheral neuropathy or occlusive vascular disease; 6. Severe systemic disease rendering the patient unable to tolerate anesthesia or surgery (ASA classification > III); 7. Pregnancy or lactation; 8. The presence of psychiatric disorders, cognitive impairment, or anticipated poor postoperative compliance precluding cooperation with follow-up. |
