研究目的：

本研究旨在评估在标准化围术期管理下，肺内振荡联合肺扩张技术相较于常规治疗，能否降低择期心脏手术患者术后肺部并发症的发生风险。  

Objectives of Study：

To evaluate whether adding oscillation and lung expansion therapy to standard postoperative care reduces the incidence of perioperative pulmonary complications in patients undergoing elective cardiac surgery.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.年龄 ≥ 18 岁； 2.接受择期心脏手术（CABG，瓣膜手术，CABG+瓣膜手术）； 3.加泰罗尼亚外科手术患者呼吸风险评估量表 (Assess Respiratory Risk in Surgical Patients in Catalonia, ARISCAT) 评分高风险 (≥45分) 。

Inclusion criteria

1. Age >= 18 years. 2. Scheduled for elective cardiac surgery (coronary artery bypass grafting [CABG], cardiac valve surgery, or combined CABG and valve surgery). 3. High perioperative respiratory risk (Assess Respiratory Risk in Surgical Patients in Catalonia [ARISCAT] score >= 45).

排除标准：

1.体质指数 (BMI) > 40 kg/m2； 2.已存在的神经肌肉疾病； 3.未处理的张力性气胸； 4.活动性肺内出血； 5.左心室射血分数 < 35%； 6.顽固性低血压或心律失常； 7.计划在术后 48 小时内接受再次手术； 8.严重呼吸系统基础疾病需要长期家庭氧疗或无创通气 9.严重的其他器官功能障碍预计短期内死亡 (7天) 或姑息性治疗； 10.妊娠 11.未签署知情同意书。

Exclusion criteria：

1. Body mass index (BMI) > 40 kg/m2. 2. Pre-existing neuromuscular disease. 3. Untreated tension pneumothorax. 4. Active pulmonary hemorrhage. 5. Left ventricular ejection fraction (LVEF) < 35%. 6. Refractory hypotension or arrhythmias. 7. Planned reoperation within 48 hours. 8. Severe underlying respiratory disease requiring long-term home oxygen therapy or noninvasive ventilation. 9. Severe dysfunction of other organs with expected short-term (7-day) mortality or palliative care needs. 10. Pregnancy. 11. Failure to provide informed consent.
