研究目的：

基于二元疾病管理理论，文献回顾、质性研究、专家会议以及预实验，构建科学、系统的老年髋部脆性骨折患者及照顾者的出院准备服务方案。验证基于二元疾病管理理论的老年髋部脆性骨折患者出院准备服务方案对提升患者康复锻炼依从性、改善功能预后以及减轻照顾者照顾负担、促进二者关系的效果。  

Objectives of Study：

Based on the dual disease management theory, a scientific and systematic discharge preparation service program for elderly patients with hip fragility fractures and their caregivers was developed through literature review, qualitative research, expert meetings, and preliminary experiments. The study aims to verify the effectiveness of this discharge preparation service program based on dual disease management theory in improving patients' adherence to rehabilitation exercises, enhancing functional prognosis, reducing caregiver burden, and promoting the relationship between the two.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

患者 1.根据《骨质疏松性骨折诊疗指南》中的相关诊断标准，确诊为髋部脆性骨折； 2.患者年龄≥60岁； 3.意识清晰且具有正常的沟通能力； 4.患者对本研究知情同意，且自愿参与并签署知情同意书； 照顾者 1.年龄大于 18 岁； 2.患者指定的主要照顾者； 3.参与患者医疗决策； 4.知情同意参与本次研究。

Inclusion criteria

Patient 1. Diagnosed with a hip fragility fracture according to the relevant diagnostic criteria in the Osteoporosis Fracture Diagnosis and Treatment Guidelines; 2. Age 60 or older; 3. Conscious and able to communicate normally; 4. Informed about this study, voluntarily participating, and has signed the consent form. Caregiver 1. Over 18 years old; 2. The primary caregiver designated by the patient; 3. Involved in the patient's medical decisions; 4. Informed and agrees to participate in this study.

排除标准：

患者 1.合并患有精神及认知障碍； 2.合并患有重要器官功能异常，如心、肝、肾等； 3.患有终末期疾病； 照顾者 1.合并严重听力或视力障碍等无法交流者

Exclusion criteria：

Patients: 1. Also have mental and cognitive disorders; 2. Also have major organ dysfunctions, such as heart, liver, or kidney issues; 3. Have terminal illnesses. Caregivers: 1. Have severe hearing or vision impairments that make communication impossible.
