研究目的：

观察比较并系统分析各组患者的一般临床指标、血清指标、粪便肠道菌群和代谢产物、结肠黏膜指标和肝纤维化程度，探究MAFLD患者结肠黏膜内IL-25水平、肠道菌群和粪便短链脂肪酸情况和肝脂肪变及肝硬度的相关性  

Objectives of Study：

Observing, comparing, and systematically analyzing general clinical indicators, serum markers, fecal gut microbiota composition and metabolites, colonic mucosal markers, and liver fibrosis severity across patient groups to investigate the correlation between colonic mucosal IL-25 levels, gut microbiota composition, fecal short-chain fatty acid (SCFA) profiles, and the degree of hepatic steatosis and liver stiffness in MAFLD patients.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.对照组：不符合MAFLD诊断标准（腹部超声或FibroScan CAP值<238 dB/m）；代谢正常（BMI<25 kg/m^2，空腹血糖<6.1 mmol/L，TG<1.7 mmol/L，HDL>=1.0 mmol/L（男）/>=1.3 mmol/L（女））； 2.转氨酶正常的MAFLD组：符合MAFLD诊断标准（肝脂肪变影像学证据+超重/代谢综合征）；肝功能正常（ALT<=40 U/L）； 3.转氨酶异常MAFLD组：符合MAFLD诊断标准；ALT>=2倍ULN（ALT>80 U/L）；

Inclusion criteria

1. Control group: Does not meet the MAFLD diagnostic criteria (abdominal ultrasound or FibroScan CAP value <238 dB/m); metabolically normal (BMI <25 kg/m^2, fasting blood glucose <6.1 mmol/L, TG <1.7 mmol/L, HDL >=1.0 mmol/L for men / >=1.3 mmol/L for women); 2. MAFLD group with normal transaminases: Meets the MAFLD diagnostic criteria (imaging evidence of liver fat, overweight/metabolic syndrome); liver function is normal (ALT <= 40 U/L); 3. MAFLD group with abnormal transaminases: Meets the MAFLD diagnostic criteria; ALT >= 2 times the upper limit of normal (ALT > 80 U/L);

排除标准：

1.对照组：酗酒（男性>20g/d，女性>10g/d）或近期饮酒史；慢性肝病史（病毒性肝炎、自身免疫性肝病、药物性肝损伤、肝内胆管结石等，可以纳入肝囊肿和肝血管瘤）；合并肠道疾病（炎症性肠病、肠肿瘤、急性感染）；免疫性疾病（类风湿关节炎、系统性红斑狼疮等）；恶性肿瘤或使用免疫抑制剂/抗生素（近3个月）；
2.转氨酶正常的MAFLD组：酗酒（男性>20g/d，女性>10g/d）或近期饮酒史；慢性肝病史（病毒性肝炎、自身免疫性肝病、药物性肝损伤、肝内胆管结石等，可以纳入肝囊肿和肝血管瘤）；合并肠道疾病（炎症性肠病、肠肿瘤、急性感染）；免疫性疾病（类风湿关节炎、系统性红斑狼疮等）；恶性肿瘤或使用免疫抑制剂/抗生素（近3个月）；
3.转氨酶异常MAFLD组：酗酒（男性>20g/d，女性>10g/d）或近期饮酒史；慢性肝病史（病毒性肝炎、自身免疫性肝病、药物性肝损伤、肝内胆管结石等，可以纳入肝囊肿和肝血管瘤）；合并肠道疾病（炎症性肠病、肠肿瘤、急性感染）；免疫性疾病（类风湿关节炎、系统性红斑狼疮等）；恶性肿瘤或使用免疫抑制剂/抗生素（近3个月）；

Exclusion criteria：

1.Control group: Alcohol abuse (males >20 g/day, females >10 g/day) or recent history of alcohol consumption; history of chronic liver disease (viral hepatitis, autoimmune liver disease, drug-induced liver injury, intrahepatic bile duct stones, etc.; hepatic cysts and hemangiomas are not excluded); concurrent intestinal diseases (inflammatory bowel disease, intestinal tumors, acute infection); immune diseases (rheumatoid arthritis, systemic lupus erythematosus, etc.); malignant tumors or use of immunosuppressants/antibiotics (within the last 3 months);
2.MAFLD Group with Normal Transaminases:Alcohol abuse (males >20 g/day, females >10 g/day) or recent history of alcohol consumption; history of chronic liver disease (viral hepatitis, autoimmune liver disease, drug-induced liver injury, intrahepatic bile duct stones, etc.; hepatic cysts and hemangiomas are not excluded); concurrent intestinal diseases (inflammatory bowel disease, intestinal tumors, acute infection); immune diseases (rheumatoid arthritis, systemic lupus erythematosus, etc.); malignant tumors or use of immunosuppressants/antibiotics (within the last 3 months);
