Objectives of Study:

This study systematically summarizes the characteristic capillary alterations associated with various dermatological conditions and develops an intelligent diagnostic system to facilitate noninvasive and rapid clinical evaluation of these diseases.

纳入标准:

1.病例组定义:患有存在微循环受累或毛细血管改变的皮肤疾病,需进行或拟行毛细血管镜检查;
2.对照组定义:健康志愿者,无明确皮肤疾病或系统性疾病相关表现;
3.影像图片拍摄清晰;
4.疾病诊疗资料完备;

Inclusion criteria

1.Case group definition: Patients with skin diseases involving microcirculatory impairment or capillary changes, who require or are scheduled to undergo capillaroscopy;
2.Control group definition: Healthy volunteers without any signs of skin disease or systemic disorders;
3.Images clearly captured;
4.Complete medical and diagnostic records for the condition.

排除标准:

1.图片质量不佳,包括模糊、对焦不清、曝光过度等;
2.工作职业可能导致手外伤;
3.检查近三周手指受到磨损(如美甲等);

Exclusion criteria:

1.The image quality is poor, including being blurry, having unclear focus, and overexposed.
2.The occupation one has may lead to hand injuries.
3.Check for any wear and tear on the fingers in the past three weeks (such as from manicures, etc.);

The capillaries around the nails of the patients were examined, recorded and analyzed using digital video capillary microscopy. If any of the characteristics such as dilation, tortuosity, abnormal morphology, disordered arrangement, or micro-hemorrhagic points were present in the capillaries around the nails of the patients, it was considered that there were abnormal characteristics of the nailfold capillaries.

数据采集和管理(说明:数据采集和管理由两部分组成,一为病例记录表(Case Record Form, CRF),二为电子采集和管理系统(Electronic Data Capture, EDC),如ResMan即为一种基于互联网的EDC:

本研究的数据采集和管理流程严格遵循院内信息安全规范,依托医院信息系统进行闭环管理。 在病例记录表方面,所有临床数据(包括患者的人口学信息、主诉、病史及随访信息)均通过结构化的电子病例记录表进行采集,门诊或病房工作站直接录入医院信息系统,系统内置逻辑核查与范围校验功能以确保数据的准确性与完整性,所有录入和修改操作均自动记录时间戳及操作者工号,便于全程溯源。 在电子采集和管理系统方面,本研究的皮肤毛细血管影像使用协和皮肤影像中心内部署的“数字视频毛细血管镜”及其配套采集软件完成,该设备及软件均为本地化部署,不具备云端同步或自动外传功能,从源头杜绝数据外泄风险。采集后的影像数据通过医院内部封闭式医疗内网,与患者基本信息和就诊病历实时匹配并上传至协和医院内部权属的“多维影像管理平台”,该平台服务器架设于医院数据中心,属于医院信息系统的核心组成部分。整个采集、传输和存储过程均在院内网络环境下闭环运行,实现物理级数据隔离,严格维护数据安全与患者隐私。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture:

The data collection and management process of this study strictly followed the hospital's information security norms and was managed in a closed loop through the hospital information system.In terms of case record forms, all clinical data (including the patient's demographic information, chief complaint, medical history, and follow-up information) were collected through structured electronic case record forms. The outpatient or ward workstations directly entered the hospital information system, and the system had built-in logical verification and range checking functions to ensure the accuracy and completeness of the data. All input and modification operations were automatically recorded with timestamps and operator ID numbers, facilitating full traceability throughout the process.In the electronic collection and management system, the skin capillary images of this study were completed using the "Digital Video Capillary Microscope" deployed by the Peking Union Medical College Hospital's Skin Imaging Center and its accompanying collection software. This equipment and software were all deployed locally and did not have cloud synchronization or automatic external transmission functions, effectively eliminating the risk of data leakage at the source. The collected image data were uploaded through the hospital's closed medical internal network to the "Multi-Dimensional Imaging Management Platform" within Peking Union Medical College Hospital, which was hosted on the hospital's data center and was a core component of the hospital information system. The entire collection, transmission, and storage process was closed-looped within the hospital network environment, achieving physical-level data isolation and strictly maintaining data security and patient privacy.
