研究目的：

1.构建基于3-PR模式的ASO高危人群健康管理方案。 2.验证健康管理方案在社区ASO高危人群中的应用效果，以期提高该人群的疾病认知水平，改变其不健康行为，预防ASO的发生与发展。  

Objectives of Study：

1. Develop a health management program for high-risk ASO populations based on the 3-PR model. 2. Verify the application effect of the health management program in high-risk ASO populations in the community, with the aim of improving this population's disease awareness, changing their unhealthy behaviors, and preventing the occurrence and progression of ASO.

纳入标准：

1. 下肢动脉硬化闭塞症高危人群：年龄≥40岁，确诊≥1项下肢动脉硬化闭塞症危险因素的患者（每天吸烟≥1支且持续1年以上、高血压病、糖尿病或高血脂）； 2. 踝肱指数≤0.99； 3. 意识清楚，能正确表达自己的意愿； 4. 能使用智能移动设备； 5. 知情同意，自愿参与本研究。

Inclusion criteria

1. High-risk groups for lower limb arteriosclerosis obliterans: patients aged ≥40 years who are diagnosed with ≥1 risk factor for lower limb arteriosclerosis obliterans (smoking ≥1 cigarette per day for more than 1 year, hypertension, diabetes, or hyperlipidemia); 2. Ankle-brachial index <= 0.99; 3. Conscious and able to correctly express their own wishes; 4. Able to use smart mobile devices; 5. Informed consent and voluntary participation in this study.

排除标准：

1. 行动不便，生活无法自理； 2. 患有精神疾病； 3. 合并有其他严重疾病或肿瘤，正处于危重状态； 4. 伴有严重的认知障碍或听觉障碍。

Exclusion criteria：

1. Mobility is impaired and unable to care for oneself; 2. Suffering from mental illness; 3. Combined with other serious diseases or tumors, in a critical condition; 4. Accompanied by severe cognitive or hearing impairments.

在对照组的基础上，实施基于3-PR模式的健康管理方案。 （1）信息：即普及知识阶段。向ASO高危人群提供信息支持，使其初步了解ASO。该阶段共4个主题讲座，讲座以PPT、视频和图文形式呈现，每个讲座结束后开展3-PR参与式活动（互动式情景剧），最后发放ASO知识清单，单次活动时间控制在30min以内。 （2）动机：即动机激发阶段。采用动机性访谈的形式，该阶段共4次访谈，每次访谈循序渐进，激发研究对象访预防ASO健康管理动机。单词访谈时间控制在30min以内。 （3）行为技巧:即行为促进阶段。指导研究对象预防ASO的行为技巧，促进其健康行为产生。本阶段针对ASO预防饮食、运动、戒烟、控制原发病、ASO早期识别分为4个主题。每个主题由以下三部分组成：参与式研究（PPT形式主题讲座），参与式扮演（组织研究对象参与当日主题相应的互动活动），参与式总结（每周发送群打卡追踪计划完成情况）。单次主题活动时间控制在30min以内。

Intervention：

Based on the control group, implement a health management program based on the 3-PR model. (1) Information: This is the knowledge dissemination phase. Provide information support to high-risk ASO populations to give them a preliminary understanding of ASO. This phase consists of 4 themed lectures, presented in the form of PPT, videos, and text with images. After each lecture, a 3-PR participatory activity (interactive role-play) is conducted, and an ASO knowledge checklist is distributed. Each session is kept within 30 minutes. (2) Motivation: This is the motivation activation phase. Motivational interviewing is used. This phase includes 4 interviews, each conducted progressively to stimulate the research participants' motivation for preventive ASO health management. Each interview is kept within 30 minutes. (3) Behavioral Skills: This is the behavior promotion phase. Guide participants in behavior skills for ASO prevention to promote healthy behavior. This phase is divided into 4 themes: diet, exercise, smoking cessation, control of primary diseases, and early identification of ASO. Each theme consists of three parts: participatory study (themed lecture in PPT form), participatory role-play (organizing participants to engage in interactive activities corresponding to the day's theme), and participatory summary (weekly group check-in to track plan completion). Each thematic activity is kept within 30 minutes.

采用目的抽样法，选取河南省洛阳市某两所社区符合纳排标准的人群作为研究对象。这两个社区属于同一社区管理服务 中心管理，管理模式相同，公共设施、人口规模相近，居民以退休职工为主，文化程度以及收入水平相似，社区间具有可比性。不参与干预实施的研究人员将两个社区分别编号为1和2，通过抽签法选出的第一个编号为干预组，另一个则为对照组。

Using purposive sampling, two communities in Luoyang, Henan Province, were selected as the study subjects, with people meeting the inclusion and exclusion criteria. These two communities are managed by the same community management service center, have the same management model, similar public facilities and population size, with residents mainly retired employees, comparable education levels, and income levels, making the communities comparable. Researchers not participating in the intervention assigned numbers 1 and 2 to the two communities, and the first number drawn by lot was designated as the intervention group, while the other as the control group.
