研究目的:

筛选并鉴定与主动脉瓣膜钙化相关的关键分子诊断标志物;探索这些候选标志物在不同细胞亚群、组织空间及特定化学成分区域(如特定脂质沉积区、异常矿化前沿)中的表达特征,分析其与疾病严重程度的关系。  

Objectives of Study:

This study aims to screen and identify key molecular diagnostic biomarkers associated with aortic valve calcification, to explore the expression characteristics of these candidate biomarkers in different cell subpopulations, tissue spaces, and specific chemical composition regions (such as specific lipid deposition areas and the abnormal mineralization front), and to analyze their relationship with disease severity.

纳入标准:

1.病例组:签名的带日期的知情同意书; 2.病例组:符合主动脉瓣钙化的诊断标准: 依据多普勒超声心动图及二维超声心动图结果,符合以下影像学特征之一或多项: 1)主动脉瓣瓣叶不规则增厚,局部回声增强,提示钙化改变; 2)瓣叶僵硬,瓣膜交界可见融合,瓣膜开口受限; 3)连续波多普勒显示跨瓣峰值血流速度正常或仅轻度升高(<2.0 m/s),可伴有跨瓣 压差增大、瓣口面积减少; 4)主动脉瓣环处可见局限性斑块样反射增强,其回声强度≥主动脉根部回声反射; 3.病例组:且符合主动脉瓣膜置换术手术指征并行手术者(符合以下任一情况): 1)钙化性主动脉瓣狭窄患者,瓣口面积≤0.75 cm2,且跨瓣压差≥6.7 kPa(50 mmHg), 合并左心室射血分数 <0.50,或伴频发晕厥、心绞痛等症状; 2)有症状的重度主动脉瓣狭窄患者,或重度主动脉瓣狭窄合并左心室收缩功能不全(射 血分数<0.50); 3)无明显症状但存在极重度主动脉瓣狭窄者(瓣口面积<0.6 cm2,平均跨瓣压差>60 mmHg,瓣口流速>5 m/s),且预计手术死亡率<1%; 4)存在瓣膜病变快速进展高风险因素(如高龄、显著钙化、合并冠心病)的主动脉瓣 狭窄患者,包括部分无症状的重度主动脉瓣狭窄患者; 5)因冠状动脉疾病需行冠状动脉旁路移植术,且合并轻度主动脉瓣狭窄,但有证据提 示瓣膜病变可能快速进展(如存在中至重度瓣膜钙化)。 4.对照组:符合主动脉瓣脱垂行瓣膜置换术标准并行手术 : 1)主动脉瓣中重度度脱垂符合瓣膜置换修复标准; 2)无以上主动脉瓣钙化病理特征人群; 5.对照组:或符合心脏移植术标准的受体患者; 6.对照组:签名的带日期的知情同意书;

Inclusion criteria

1.Case group: Signed and dated informed consent form; 2.Case group: Meet the diagnostic criteria for aortic valve calcification: Based on Doppler echocardiography and two-dimensional echocardiography findings, meeting one or more of the following imaging features: 1) Irregular thickening of the aortic valve leaflets with locally enhanced echoes, suggesting calcific changes; 2) Stiff leaflets with visible commissural fusion and restricted valve opening; 3) Continuous-wave Doppler showing normal or only mildly elevated peak transvalvular flow velocity (<2.0 m/s), possibly accompanied by increased transvalvular pressure gradient and reduced valve orifice area; 4) Localized plaque-like echo enhancement at the aortic valve annulus, with echo intensity >= that of the aortic root echo reflection; 3.Case group: And meet the surgical indications for aortic valve replacement (meeting any of the following conditions): 1) Patients with calcific aortic stenosis, aortic valve area <= 0.75 cm2, and transvalvular pressure gradient >= 6.7 kPa (50 mmHg), accompanied by left ventricular ejection fraction < 0.50, or frequent syncope, angina, or other symptoms; 2) Symptomatic patients with severe aortic stenosis, or severe aortic stenosis combined with left ventricular systolic dysfunction (ejection fraction < 0.50); 3) Asymptomatic patients with very severe aortic stenosis (aortic valve area < 0.6 cm2, mean transvalvular pressure gradient > 60 mmHg, peak transvalvular velocity > 5 m/s) and an expected surgical mortality rate < 1%; 4) Patients with aortic stenosis who have high-risk factors for rapid progression of valve disease (e.g., advanced age, significant calcification, concomitant coronary artery disease), including some asymptomatic patients with severe aortic stenosis; 5) Patients requiring coronary artery bypass grafting for coronary artery disease and presenting with mild aortic stenosis, but with evidence suggesting possible rapid progression of valve disease (e.g., moderate to severe valve calcification). 4.Control group: Meet the criteria for valve replacement due to aortic valve prolapse and undergo surgery: 1) Moderate to severe aortic valve prolapse meets the criteria for valve replacement/repair; 2) Individuals without the above-mentioned pathological features of aortic valve calcification; or recipients who meet the criteria for heart transplantation; 5.Control group: or recipients who meet the criteria for heart transplantation. 6.Control group: Signed and dated informed consent form;

排除标准:

1.风湿性心脏病导致的瓣膜病变; 梅毒、大动脉炎;
2.感染性心内膜炎或其他活动性、急性炎症性疾病;
3.梅毒、大动脉炎;
4.马凡综合征或其他明确诊断的结缔组织疾病;
5.临床资料或影像学资料不完整,无法满足研究分析要求者。

Exclusion criteria:

1. Valvular lesions caused by rheumatic heart disease; 2. Infective endocarditis or other active, acute inflammatory diseases; 3. Syphilis, Takayasu's arteritis; 4. Marfan syndrome or other definitively diagnosed connective tissue diseases; 5. Cases where clinical or imaging data are incomplete and cannot meet the requirements for research analysis.
