研究目的：

目的： 1.规避经胆总管切开胆道镜探查取石T管引流手术的缺点和不足； 2.简化手术流程； 3.缩短手术时间； 4.减少术中出血量； 5.降低术后并发症； 6.避免患者长时间携带T管引流导致的疼痛、T管脱落、阻塞、胆汁渗入腹腔、感染等一系列问题和T管相关的并发症。  

Objectives of Study：

Objective: 1. To avoid the drawbacks and shortcomings of the exploration and removal of stones through cholangioscopy and T-tube drainage surgery via the common bile duct; 2. Simplify the surgical process; 3. Shorten the surgical time; 4. Reduce intraoperative bleeding; 5. Reduce postoperative complications; 6. To avoid a series of problems and complications related to T-tube drainage, such as pain, T-tube detachment, blockage, bile infiltration into the abdominal cavity, infection, etc. caused by patients carrying T-tube drainage for a long time.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.肝内胆管结石合并肝外胆管结石：经腹部B超、CT或MRCP检查提示肝内胆管结石合并肝外胆管结石； 2.存在手术适应证，无手术禁忌证，行腹腔镜下肝切除联合术中胆道镜探查取石术的手术治疗； 3.既往无经内镜逆行性胰胆管造影术(ERCP)检查及治疗史。

Inclusion criteria

1. Intrahepatic bile duct stones combined with extrahepatic bile duct stones: Abdominal ultrasound, CT or MRCP examination suggests intrahepatic bile duct stones combined with extrahepatic bile duct stones; 2. There are indications for surgery, but no contraindications. Laparoscopic liver resection combined with intraoperative cholangioscopy exploration and stone removal surgery is performed for surgical treatment; 3. No previous history of endoscopic retrograde cholangiopancreatography (ERCP) examination or treatment.

排除标准：

1.合并患有肝脏或胆道恶性肿瘤等其他部位恶性肿瘤者； 2.合并患有急性胆管炎或急性胰腺炎者； 3.合并患有肝外胆管严重狭窄者； 4.合并严重心肺脑等疾病无法耐受手术； 5.严重凝血功能障碍者。

Exclusion criteria：

1. Patients with concurrent malignant tumors of the liver or biliary tract or other parts of the body; 2. Patients with concurrent acute cholangitis or acute pancreatitis; 3. Patients with severe stenosis of extrahepatic bile ducts; 4. Patients with severe cardiovascular, pulmonary, and cerebrovascular diseases cannot tolerate surgery; 5. Individuals with severe coagulation dysfunction.
