研究目的：

本研究旨在建立肥胖与代谢异常相关主动脉疾病研究队列，系统收集人口学资料、临床特征、实验室指标、影像学信息及外周血、主动脉组织、主动脉周围脂肪组织等生物样本，评估肥胖及代谢异常与主动脉疾病发生、进展及预后的关系，重点明确肥胖状态、代谢状态及联合分型与主动脉疾病临床表型、影像学表现、病情进展和临床结局之间的关联，并结合手术标本子队列中血液、主动脉组织及主动脉周围脂肪组织的检测结果，初步探索肥胖及代谢紊乱通过局部炎症反应、血管壁重塑及主动脉周围脂肪组织微环境变化参与主动脉疾病发生发展和不良预后的潜在机制，为主动脉疾病高危人群识别、早期风险评估及防治策略优化提供依据。  

Objectives of Study：

This study aims to establish a research cohort of obesity- and metabolic abnormality–related aortic diseases. Demographic data, clinical characteristics, laboratory indicators, imaging information, and biological samples—including peripheral blood, aortic tissue, and perivascular adipose tissue—will be systematically collected.The study will evaluate the associations of obesity and metabolic abnormalities with the occurrence, progression, and prognosis of aortic diseases. In particular, it will clarify the relationships between obesity status, metabolic status, and their combined phenotypes with the clinical phenotypes, imaging features, disease progression, and clinical outcomes of aortic diseases. Furthermore, by integrating findings from blood, aortic tissue, and perivascular adipose tissue analyses in the surgical specimen subcohort, this study will preliminarily explore the potential mechanisms through which obesity and metabolic dysregulation contribute to the development, progression, and adverse outcomes of aortic diseases, including local inflammatory responses, vascular wall remodeling, and alterations in the perivascular adipose tissue microenvironment. The findings are expected to provide evidence for identifying high-risk populations, improving early risk assessment, and optimizing prevention and treatment strategies for aortic diseases.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1）年龄≥18岁； （2）经CTA或术中探查明确诊断为主动脉疾病； （3）具备可获取的基线临床资料、影像学资料及必要的实验室指标； （4）前瞻性队列受试者能够理解研究内容并签署知情同意书，并同意随访； （5）手术标本子队列纳入条件：在满足上述纳入标准基础上，接受主动脉相关手术治疗且术中可获取合格标本者。

Inclusion criteria

(1)Age >=18 years; (2)Definite diagnosis of aortic disease by computed tomography angiography (CTA) or intraoperative findings; (3)Availability of baseline clinical data, imaging data, and essential laboratory indicators; (4)For participants enrolled in the prospective cohort: ability to understand the study procedures, provide written informed consent, and agree to follow-up; (5)For participants enrolled in the surgical specimen subcohort: in addition to meeting the above inclusion criteria, patients must undergo aortic-related surgery and have eligible intraoperative specimens available for collection.

排除标准：

（1）明确的遗传性主动脉疾病； （2）感染性、创伤性、医源性或炎症性血管炎相关主动脉病变； （3）合并活动性恶性肿瘤、活动性感染、严重免疫系统疾病或长期接受全身性免疫抑制治疗； （4）临床资料严重缺失者； （5）前瞻性队列中预计无法完成随访或依从性极差者。

Exclusion criteria：

(1)Definite hereditary aortic disease; (2)Infectious, traumatic, iatrogenic, or inflammatory vasculitis-related aortic lesions; (3)Presence of active malignancy, active infection, severe immune system disease, or long-term systemic immunosuppressive therapy; (4)Severe missingness of clinical data; (5)For participants enrolled in the prospective cohort: anticipated inability to complete follow-up or extremely poor compliance.

From baseline enrollment to the end of follow-up. In the prospective cohort, follow-up is scheduled at 30 days, 3 months, 6 months, and 12 months after discharge, and annually thereafter.

Measure method：

Major adverse aortic events are defined as a composite outcome including aortic-related death, aortic rupture, aortic-related reintervention, and radiologically confirmed progression of aortic disease. Individual components of the composite outcome will be analyzed separately as secondary outcomes.
