研究目的:

本研究旨在评估牙周非手术治疗(NSPT)干预对重度牙周炎患者口腔及肠道菌群结构的重塑作用。借助菌群定植小鼠模型,评估并验证不同来源菌群的全身致病能力差异,探索牙周炎菌群介导全身疾病的关键生物学基础。  

Objectives of Study:

This study aimed to evaluate the remodeling effect of non-surgical periodontal therapy (NSPT) on the structural composition of oral and intestinal microbiota in patients with severe periodontitis. Using a microbiota colonization mouse model, we assessed and verified differences in the systemic pathogenicity of microbiota from different sources, and explored the key biological basis by which periodontitis-associated microbiota mediates systemic diseases.

Description for medicine or protocol of treatment in detail:

 

纳入标准:

1.年龄在 20~65 岁间,存留牙齿>=10 颗; 2.BMI 值>=18.5,<=30; 3.身体健康,无糖尿病、胃肠道疾病; 4.非孕妇、哺乳期女性,当前未吸烟者; 5.过去 6 个月内无牙周治疗; 6.过去 3 个月内无抗生素使用; 7.牙周炎诊断标准同时满足以下 4 个条件:(1)最严重的患牙邻面附着丧失(clinical attachment loss ,CAL)>=5mm,探诊深度(probing depth ,PD)>=6mm;(2)影像学骨丧失至根中 1/3 或根尖 1/3;(3)因牙周炎失牙<=4 颗;(4)无咬合功能障碍、继发性牙牙合创伤、咬合紊乱、牙移位等需复杂修复重建。

Inclusion criteria

1. Age between 20 and 65 years old, with at least 10 remaining teeth; 2. BMI value between 18.5 and 30; 3. In good health, without diabetes or gastrointestinal diseases; 4. Not pregnant or breastfeeding, and currently not a smoker; 5. No periodontal treatment in the past 6 months; 6. No antibiotic use in the past 3 months; 7. The diagnosis criteria for periodontitis simultaneously meet the following 4 conditions: (1) The most severe affected tooth has a clinical attachment loss (CAL) of >= 5mm and a probing depth (PD) of >= 6mm; (2) Radiographic bone loss reaches the middle 1/3 or apical 1/3 of the root; (3) Tooth loss due to periodontitis is <= 4 teeth; (4) There is no occlusal dysfunction, secondary occlusal trauma, occlusal disorder, tooth displacement, etc. that require complex restoration and reconstruction.

排除标准:

1.有全身系统性疾病,如糖尿病、胃肠道疾病; 2.孕妇、哺乳期女性,当前吸烟者; 3.过去 6 个月内行牙周治疗; 4.过去 3 个月内使用抗生素

Exclusion criteria:

1.Presence of systemic diseases, such as diabetes mellitus and gastrointestinal diseases.
2.Pregnant or lactating women; current smokers.
3.Received periodontal treatment within the past 6 months.
4.Used antibiotics within the past 3 months.

测量方法:

采用标准化牙周检查方法,由 2 名经校准的牙周专科医师完成测量。使用牙周探针检查全口牙齿,每牙检查 6 个位点:颊侧近中、中央、远中,舌侧近中、中央、远中,记录牙菌斑指数(PI)、探诊出血(BOP)、临床附着水平(CAL)、牙周袋深度(PD),取平均值进行统计分析。

Measure method:

Periodontal measurements were performed by two calibrated periodontists using a standardized periodontal probe. Plaque Index (PI), Bleeding on Probing (BOP), Clinical Attachment Level (CAL), and Probing Depth (PD) were recorded at six sites per tooth: mesiobuccal, midbuccal, distobuccal, mesiolingual, midlingual, and distolingual. Mean values were calculated for statistical analysis.
