研究目的：

主要目的： 通过随机对照临床研究，定量评估青少年上颌骨横向发育不足患者在接受不同扩弓治疗方式前后颧骨间宽度的变化，明确不同扩弓治疗方式对面部骨性横向表型特征的影响程度，从而为扩弓治疗在改善面部表型方面的临床价值提供客观循证依据。 次要目的： 评估不同扩弓治疗方式对鼻翼宽度的影响； 评估不同扩弓治疗方式对上颌骨宽度及上下颌骨矢状向生长发育变化的影响，探讨扩弓干预对颌骨发育的潜在作用； 分析不同扩弓治疗方式前后面部表型指标的变化，探究扩弓治疗对面部表型的潜在作用效果； 分析不同扩弓治疗方式前后上气道体积、睡眠监测指标的变化，明确扩弓治疗对上气道体积及睡眠呼吸功能的改善效果； 分析不同扩弓治疗方式前后口腔健康相关生活质量、心理量表评分的变化及患者满意度/舒适度。  

Objectives of Study：

Primary objective: To quantitatively assess interzygomatic width changes following different maxillary expansion treatments in adolescents with transverse maxillary deficiency via an RCT, and to determine the impact on facial skeletal transverse phenotype, providing evidence for the clinical value of expansion in improving facial phenotype. Secondary objectives: To assess the effect on nasal alar width. To assess the effect on maxillary width and sagittal jaw growth, exploring the role of expansion on jaw development. To analyze changes in facial phenotype indices and explore the effect of expansion on facial phenotype. To analyze changes in upper airway volume and sleep monitoring parameters, clarifying the improvement effect on airway volume and sleep respiratory function. To analyze changes in oral health-related quality of life, psychological scale scores, and patient satisfaction/comfort.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1. 年龄9岁<=年龄<=15岁，性别不限； 2. 临床诊断为上颌横向发育不足； 3. 上颌第一磨牙及前磨牙已萌出； 4. 患者及监护人知情同意。

Inclusion criteria

1. Age 9 years <= age <= 15 years, gender not limited; 2. The clinical diagnosis is insufficient lateral development of the maxilla; 3. The maxillary first molars and premolars have erupted; 4. Informed consent from the patient and their guardian.

排除标准：

1. 颌面部外伤或先天畸形史； 2. 严重全身性及遗传性疾病； 3.不能配合治疗者。

Exclusion criteria：

1. History of maxillofacial trauma or congenital malformation; 2. Severe systemic or genetic diseases; 3. Inability to cooperate with treatment.

佩戴Hass或Hyrax扩弓器，分别粘接于上颌第一恒磨牙和第一前磨牙，以每日0.5mm的速度快速扩弓，达到相同目标扩弓量后停止扩弓并保持6个月。

Patients will wear either a Hass or Hyrax expander, bonded to the maxillary first permanent molars and first premolars respectively. Rapid maxillary expansion will be performed at a rate of 0.5 mm per day. Once the same target expansion amount is achieved, active expansion will be stopped, followed by a 6-month retention period.
