本研究旨在通过一项回顾性队列研究,系统分析 2020 年至 2025 年间,规律运动与体力活动对上海市社区患有高血压、高血脂、高血糖、冠心病及超重/肥胖的中老年人群产生的综合影响。研究将基于社区运动数据、体质健康档案、体检数据及医疗保障结算数据等,探讨以下方面:①评估运动与该人群健康指标的关联,包括对血压、血糖、血脂等体质情况,心肺耐力、肌肉力量等体适能水平,以及体脂率、腰围等身体成分的影响;②分析运动对生化指标(如糖化血红蛋白、肝肾功能标志物)的调节作用。③量化评估不同运动水平对医疗经济负担的影响,核心指标为医保费用支出、慢病相关住院率与费用,从而揭示运动在改善健康结局与降低直接医疗成本方面的潜在价值,为社区精准化慢病运动干预和卫生资源优化配置提供循证依据。  

Objectives of Study:

This study aims to conduct a systematic analysis, through a retrospective cohort study, of the comprehensive effects of regular exercise and physical activity on middle-aged and older adults in Shanghai’s communities who suffer from hypertension, hyperlipidaemia, hyperglycaemia, coronary heart disease, and overweight/obesity between 2020 and 2025. Drawing on community exercise data, physical fitness records, health examination data, and medical insurance claims data, the study will explore the following aspects: ① Assessing the association between exercise and health indicators in this population, including its impact on physiological parameters such as blood pressure, blood glucose and blood lipid levels; fitness levels such as cardiorespiratory endurance and muscle strength; and body composition indicators such as body fat percentage and waist circumference; ② Analysing the regulatory effect of exercise on biochemical indicators (e.g. glycated haemoglobin, liver and kidney function markers). ③ Quantitatively assess the impact of different levels of physical activity on the healthcare burden, with key indicators including healthcare expenditure, hospitalisation rates and costs associated with chronic diseases. This will reveal the potential value of physical activity in improving health outcomes and reducing direct healthcare costs, thereby providing evidence-based support for targeted community-based exercise interventions for chronic diseases and the optimal allocation of healthcare resources.

纳入标准:

(1)在 2020 年 1 月 1 日至 2025 年 12 月 31 日的观察期内,为上海市杨浦区社区在册管理的常住居民,且年龄在 19 周岁到 80 周岁之间,男女不限。 (2)在观察期内被社区卫生服务中心或二级及以上医院,三明医院,至少明确诊断为高血压、高血脂、2 型糖尿病、冠心病、超重/肥胖中一种。 (3)运动风险为低中风险人群,在社区健康档案或运动门诊记录中,有明确的体力活动水平记录,可被归类为‘规律运动’、‘偶尔运动’或‘不运动’的人群。 (4)身高、体重、血压等健康数据、体适能评估信息、体成分信息、医保结算数据等核心指标完整。

Inclusion criteria

1. During the observation period from 1 January 2020 to 31 December 2025, participants must be permanent residents registered with a community health centre in Yangpu District, Shanghai, aged between 19 and 80 years, regardless of gender. 2. Individuals who, during the observation period, were diagnosed by a community health service centre or a secondary-level or higher hospital (including Sanming Hospital) with at least one of the following conditions: hypertension, hyperlipidaemia, type 2 diabetes, coronary heart disease, or overweight/obesity. 3. Individuals classified as low-to-moderate risk for exercise-related issues, with clear records of physical activity levels in their community health records or exercise clinic records, and who can be categorised as 'regular exercisers', 'occasional exercisers' or 'non-exercisers'. 4. Core indicators, including health data such as height, weight and blood pressure, physical fitness assessment information, body composition data and medical insurance settlement data, are complete.

排除标准:

(1)缺乏用于确认慢病诊断、体力活动水平与模式、或核心结局指标(体成分、体适能、生化指标、医保费用)的关键数据,无法进行有效分组或分析。 (2)患有严重心脑血管事件(如心肌梗死、脑卒中后遗症重度残疾)、终末期肾病、恶性肿瘤、严重关节炎或其它影响运动能力的疾病。 (3)根据医疗记录,被明确诊断为有不适宜进行规律体力活动的医学禁忌

Exclusion criteria:

1. A lack of key data required to confirm the diagnosis of chronic conditions, levels and patterns of physical activity, or core outcome measures (body composition, physical fitness, biochemical indicators, healthcare costs), rendering effective grouping or analysis impossible. 2. Suffering from severe cardiovascular or cerebrovascular events (such as myocardial infarction or severe disability resulting from stroke), end-stage renal disease, malignant tumours, severe arthritis, or other conditions that impair physical activity capacity. 3. Individuals who, according to medical records, have been clearly diagnosed with medical contraindications rendering them unsuitable for regular physical activity.

数据采集和管理(说明:数据采集和管理由两部分组成,一为病例记录表(Case Record Form, CRF),二为电子采集和管理系统(Electronic Data Capture, EDC),如ResMan即为一种基于互联网的EDC:

采用院内临床研究数据管理系统等进行数据收集。由项目负责人指定数据管理员,经集中培训后,严格按照CRF提取2020年1月至2025年12月社区卫生服务中心电子健康档案数据,回溯性录入EDC系统。系统具备逻辑核查功能,可对关键变量范围、缺失值及异常值进行实时预警。数据管理采用双录入或数据核对机制,保证数据完整性。数据最终以Excel等格式导出供统计分析。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture:

Data collection was conducted using an in-hospital clinical research data management system, etc. The project leader designated a data manager, who, after centralized training, strictly extracted electronic health record data from community health service centers from January 2020 to December 2025 according to the CRF and retrospectively entered it into the EDC system. The system has a logical verification function that can issue real-time alerts for the range of key variables, missing values, and outliers. Data management adopted a double-entry or data verification mechanism to ensure data integrity. The final data was exported in formats such as Excel for statistical analysis.
