探讨肥胖与哮喘之间的临床关联,并进一步评估肺组织中VNN1与HIF-1表达升高在肥胖相关哮喘中的作用,结合临床病例分析与肺组织样本验证,从流行病学关联和组织分子机制两个层面,为肥胖哮喘的发生发展机制提供依据。  

Objectives of Study:

This study explores the clinical correlation between obesity and asthma, and further assesses the role of increased expression of VNN1 and HIF-1 in lung tissue in obesity-related asthma. By combining clinical case analysis and lung tissue sample verification, it provides evidence from both epidemiological association and tissue molecular mechanism perspectives to elucidate the pathogenesis of obesity-related asthma.

纳入标准:

1. 诊断与分组标准 哮喘病史组:根据《支气管哮喘防治指南(2020年版)》,由呼吸内科医生明确诊断,并有持续用药或随访记录。或术前肺功能检查显示支气管舒张试验阳性(FEV1改善率 >= 12%,且绝对值增加 >= 200 ml)。 肥胖组:体重指数(BMI)>= 28 kg/m^2(中国标准)。 分组:根据上述标准,将患者分为四组: (1) 肥胖合并哮喘组(O+A+):BMI >= 28 且有哮喘病史。 (2) 单纯肥胖组(O+A-):BMI >= 28 且无哮喘病史。 (3) 单纯哮喘组(O-A+):BMI 18.5-23.9 且有哮喘病史。 (4) 非哮喘对照组(O-A-):BMI 18.5-23.9 且无哮喘病史。 2. 纳入标准 病例纳入标准:确诊哮喘的患者,年龄范围 18 岁以上,BMI 记录完整的病例。 肺组织样本纳入标准: (1) 年龄 18-75 岁,性别不限。 (2) 因临床诊断(如早期肺癌、支气管扩张症、肺错构瘤等)计划行择期肺叶切除术。 (3) 术前能够充分理解并自愿签署知情同意书。 (4) 符合上述四组之一的分组标准。

Inclusion criteria

1. Diagnostic and Grouping Criteria Asthma History Group: According to the "Guidelines for the Prevention and Treatment of Bronchial Asthma (2020 Edition)", the diagnosis must be confirmed by a respiratory physician, with records of continuous medication or follow-up. Alternatively, preoperative lung function tests showing a positive bronchodilator test (FEV1 improvement >= 12% and an absolute increase >= 200 ml) qualify. Obesity Group: Body Mass Index (BMI) >= 28 kg/m^2 (China standard). Grouping: Based on the above criteria, patients are divided into four groups: (1) Obese with Asthma Group (O A): BMI >= 28 and a history of asthma. (2) Simple Obesity Group (O A-): BMI >= 28 and no history of asthma. (3) Simple Asthma Group (O-A): BMI 18.5-23.9 and a history of asthma. (4) Non-Asthma Control Group (O-A-): BMI 18.5-23.9 and no history of asthma. 2. Inclusion Criteria Case Inclusion Criteria: Patients with confirmed asthma, aged 18 years and above, with complete BMI records. Lung Tissue Sample Inclusion Criteria: (1) Aged 18-75 years, of any gender. (2) Scheduled for elective lobectomy due to clinical diagnosis (e.g., early lung cancer, bronchiectasis, pulmonary hamartoma, etc.). (3) Able to fully understand and voluntarily sign the informed consent form prior to surgery. (4) Meet the grouping criteria of one of the four groups mentioned above.

排除标准:

病例排除标准: 1. 年龄 < 18 岁患者; 2. 不符合纳入标准或临床资料不全者。 肺组织样本排除标准: 1. 患者或法定代理人拒绝签署知情同意书; 2. 手术取消或术中改变术式(如仅行楔形切除); 3. 合并其他活动性肺部疾病(如活动性肺结核、急性肺炎、特发性肺纤维化等); 4. 术前 3 个月内接受过全身性糖皮质激素、免疫抑制剂或放化疗(可能干扰炎症和分子标志物表达); 5. 妊娠或哺乳期妇女; 6. 研究者判断存在任何可能影响研究执行或评估的其他情况。

Exclusion criteria:

Case Exclusion Criteria: 1. Patients under 18 years of age; 2. Those who do not meet the inclusion criteria or have incomplete clinical data. Lung Tissue Sample Exclusion Criteria: 1. Patient or legal guardian refuses to sign the informed consent form; 2. Surgery canceled or intraoperative change of surgical procedure (e.g., only wedge resection performed); 3. Concurrent active lung diseases (e.g., active pulmonary tuberculosis, acute pneumonia, idiopathic pulmonary fibrosis, etc.); 4. Received systemic glucocorticoids, immunosuppressants, or radiotherapy/chemotherapy within 3 months before surgery (which may interfere with inflammation and molecular marker expression); 5. Pregnant or breastfeeding women; 6. Any other situations that the investigator believes may affect the execution or assessment of the study.

数据采集和管理(说明:数据采集和管理由两部分组成,一为病例记录表(Case Record Form, CRF),二为电子采集和管理系统(Electronic Data Capture, EDC),如ResMan即为一种基于互联网的EDC:

数据采集:临床回顾性研究,收集2010–2026年广州中医药大学第一附属医院住院哮喘患者病史资料包括收集年龄、分期、治疗方案等。数据来自电子病历系统,双人录入Excel。 在同院同期接受肺切除手术的患者中,收集非哮喘对照、单纯肥胖、单纯哮喘及肥胖合并哮喘患者的肺组织样本及临床资料。采用免疫组化、qPCR 或 Western blot 等方法检测肺组织中 VNN1 和 HIF-1α表达水平,比较四组间差异,并分析其与 BMI、哮喘病史、影像学特征及炎症指标之间的关系,以验证第一部分临床研究发现并探讨潜在机制。 数据管理:原始数据双人录入Excel,进行范围检查和逻辑核查;保存为加密文件,仅授权人员访问;患者信息去标识化处理。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture:

Data collection: This was a retrospective clinical study that collected medical records of inpatients with asthma at the First Affiliated Hospital of Guangzhou University of Chinese Medicine between 2010 and 2026, including data on age, disease stage, and treatment regimens. Data were retrieved from the electronic medical records system and entered into Excel by two researchers. Among patients who underwent lung resection at the same hospital during the same period, lung tissue samples and clinical data were collected from non-asthmatic controls, patients with obesity alone, patients with asthma alone, and patients with obesity and asthma. Methods such as immunohistochemistry, qPCR, and Western blot were employed to detect the expression levels of VNN1 and HIF-1α in lung tissue. Differences between the four groups were compared, and their relationships with BMI, asthma history, radiological features, and inflammatory markers were analysed to validate the findings from the first part of the clinical study and explore potential mechanisms. Data management: Raw data were entered into Excel by two independent operators, followed by range checks and logical verification; data were saved as encrypted files accessible only to authorised personnel; patient information was de-identified.
