研究目的：

化脓性汗腺炎（hidradenitis suppurativa, HS）是一种慢性、复发性、炎症性皮肤病，常累及腋窝、腹股沟等屈侧部位，表现为反复出现的炎性结节、脓肿、窦道及瘢痕，严重影响患者疼痛体验、日常活动和生活质量。对于中重度、反复发作且已形成窦道和瘢痕的腋窝HS，广泛切除被认为是重要的外科治疗方式之一；切除后创面修复方式则直接关系到创面愈合、并发症、复发控制、功能恢复及患者满意度。 本研究拟基于多中心既往临床数据库，开展一项多中心回顾性观察性研究，纳入接受腋窝HS广泛切除术后皮瓣修复的患者，系统评估其疗效与安全性。研究将重点分析创面愈合时间、术后并发症、复发情况及患者满意度等临床结局，并结合年龄、性别、BMI、吸烟、糖尿病、Hurley分期、病程、皮瓣类型、皮瓣面积等变量，探索影响疗效的相关因素。同时，本研究拟将所得结果与既往文献中腋窝HS广泛切除后二期愈合的报道进行审慎比较，以评价皮瓣修复在特定结局方面的潜在优势。由于HS领域长期存在结局指标异质、患者报告结局不足的问题，因此开展本研究具有较强的临床现实意义。  

Objectives of Study：

Hidradenitis suppurativa (HS) is a chronic, recurrent inflammatory skin disease predominantly affecting flexural sites such as the axillae and inguinal regions. It manifests as recurrent inflammatory nodules, abscesses, sinus tracts and scars, which severely impair patients’ pain perception, daily activities and quality of life.For moderate-to-severe, recurrent axillary HS complicated with sinus tracts and scar formation, wide excision is regarded as one of the core surgical treatments. Post-excision wound reconstruction approaches directly determine wound healing progress, complication rates, recurrence control, functional recovery and patient satisfaction.This multicenter retrospective observational study will be conducted based on historical multicenter clinical databases, enrolling patients who underwent skin flap reconstruction following wide excision for axillary HS, to systematically evaluate the clinical efficacy and safety of this therapeutic strategy. Primary clinical outcomes including wound healing time, postoperative complications, recurrence rate and patient satisfaction will be analyzed. Correlative influencing factors will be further explored by incorporating variables such as age, gender, body mass index (BMI), smoking status, diabetes mellitus, Hurley staging, disease duration, flap type and flap size.In addition, the study findings will be carefully compared with previously published data regarding secondary intention healing after wide excision for axil

纳入标准：

1.临床明确诊断为腋窝化脓性汗腺炎（HS）（Hurley Ⅱ/Ⅲ期）；
2.接受双侧腋窝广泛切除术（切除范围达深筋膜层）；
3.术后皮肤缺损采用皮瓣修复（任意皮瓣类型，如局部旋转皮瓣、V-Y推进皮瓣等）；
4.具有完整的术前、术中及术后至少6个月的随访记录；（随访过程已完成，研究仅回顾提取相关数据）；
5.数据库中基线资料（年龄、性别、BMI、吸烟史、糖尿病史、Hurley分期、切除面积等）完整；

Inclusion criteria

1.Clinically confirmed diagnosis of axillary hidradenitis suppurativa (HS) (Hurley stage II/III);
2.Undergoing bilateral extensive axillary excision (with resection extending to the deep fascial layer);
3.Postoperative skin defects repaired with flaps (any type of flap, e.g., local rotation flap, V-Y advancement flap, etc.);
4.Availability of complete preoperative, intraoperative, and postoperative follow-up records for at least 6 months (follow-up completed; study only retrospectively extracts relevant data);
5.Complete baseline data in the database, including age, sex, body mass index (BMI), smoking history, diabetes history, Hurley stage, resection area, etc.

排除标准：

1.术后缺损采用二期缝合、植皮或其他非皮瓣修复方式；
2.未接受广泛切除手术治疗患者；
3.术后失访或随访资料不完整者；（随访数据未达到术后至少6个月，或关键随访信息缺失）；
4.长期使用糖皮质激素或免疫抑制剂者；
5.合并其他可能影响创面愈合的严重疾病（如严重免疫缺陷、活动期恶性肿瘤、严重肝肾功能不全、重度营养不良等）；

Exclusion criteria：

1.Postoperative defects managed by secondary intention healing, skin grafting, or other non-flap repair methods;
2.Patients who did not undergo extensive excisional surgery;
3.Loss to follow-up or incomplete follow-up data (follow-up duration less than 6 months post-surgery, or missing key follow-up information);
4.Long-term use of glucocorticoids or immunosuppressive agents;
5.Presence of other serious diseases that may affect wound healing (e.g., severe immunodeficiency, active malignancy, severe hepatic or renal dysfunction, severe malnutrition, etc.).

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

The data of this study were derived from previous clinical databases, electronic medical record systems, surgical records and postoperative follow-up records. The research team will collect relevant materials in accordance with a unified data extraction table. Two researchers independently advanced the data and established case record forms. Each patient undergoes de-identification processing and is assigned a unique research number. The comparison table of the research number and identity information is kept separately by a designated person. Data files are stored in password-protected computers or secure servers within the hospital and are not accessible without authorization.
