研究目的:

1. 利用 Fetal HQ和多普勒技术,横向比较四组胎儿(双重阴性对照组、单纯超重/肥胖组、单纯 GDM组、双重暴露组)的心脏几何形态(GSI)及收缩功能(GLS, FAC)差异。 2. 分析孕早期BMI超重与 GDM 在导致胎儿心脏重构中是否存在交互作用(Interaction Effect)。 3. 探讨 Fetal HQ 的特征性参数(如球形指数)在不同代谢亚型中的变化规律。  

Objectives of Study:

1. Fetal HQ and Doppler technology were used to horizontally compare the differences in cardiac geometry (GSI) and systolic function (GLS, FAC) of four groups of fetuses (double negative control group, simple overweight/obesity group, simple GDM group, and double exposure group). 2. To analyze whether BMI overweight and GDM in the first trimester have an interaction effect in causing fetal cardiac remodeling. 3. To explore the changes in characteristic parameters of Fetal HQ (such as spherical index) in different metabolic subtypes.

纳入标准:

1.为超声测量胎儿各项生长指标与孕周相符,无任何已知的可识别的遗传或染色体异常; 2.所有参与者都接受了妊娠糖尿病 (GDM) 的口服糖耐力测试 (OGTT) 筛查。诊断标准基于 2014 年 8 月修订的中国标准。当妊娠 24 至 28 周期间进行的 OGTT 结果符合以下任何一个或多个标准时,GDM 被诊断: (1) 空腹血糖>= 5.1 mmol/L, (2) 1 小时血糖>=10.0 mmol/L, (3) 2 小时血糖>= 8.5 mmol/L, 如果确诊为GDM,血糖控制良好的标准为HbA1c < 6.0% ; 3.孕周为 26~30 周; 4.所有入组病例均图像清晰、目标位于图像正中、背景纯净无遮挡,可清晰显示胎儿心脏四腔心切面。

Inclusion criteria

1. Ultrasound measurements of various fetal growth indicators were consistent with gestational age, with no known recognizable genetic or chromosomal abnormalities; 2. All participants underwent an oral glucose tolerance test (OGTT) screening for gestational diabetes mellitus (GDM). The diagnostic criteria were based on the revised Chinese standards of August 2014. GDM was diagnosed if the OGTT results conducted between 24 and 28 weeks of gestation met any one or more of the following criteria: (1) Fasting blood glucose >= 5.1 mmol/L, (2) 1-hour blood glucose >= 10.0 mmol/L, (3) 2-hour blood glucose >= 8.5 mmol/L. For confirmed GDM cases, good blood glucose control was defined as HbA1c < 6.0%; 3. Gestational age of 26–30 weeks; 4. All enrolled cases had clear images, with the target positioned at the center of the image, a clean and unobstructed background, clearly showing the fetal four-chamber heart view.

排除标准:

(1)多胎妊娠; (2)母体吸烟史,饮酒史,高血压,孕前糖尿病(PGDM),心脏肝脏疾病,合并子痫前期、甲状腺功能异常、自身免疫性疾病等及其他慢性疾病; (3)胎儿心脏结构异常(如先天性心脏病等)、心律失常或染色体异常。 (4)胎儿生长受限(FGR)或巨大儿。

Exclusion criteria:

1. Multiple pregnancy; 2. Maternal history of smoking, drinking, hypertension, pregestational diabetes (PGDM), heart and liver disease, combined with preeclampsia, thyroid dysfunction, autoimmune diseases and other chronic diseases; 3. Fetal heart structure abnormalities (such as congenital heart disease, etc.), arrhythmias or chromosomal abnormalities. 4. Fetal growth restriction (FGR) or macrosomia.
