研究目的：

基于故障树分析与随机森林模型识别胸外科患者 PACU 发生拔管延迟的关键基本事件，主要风险路径以及相关变量的预测性能与重要性。  

Objectives of Study：

This study aims to identify key basic events and major risk pathways contributing to delayed extubation in thoracic surgery patients in the post-anesthesia care unit (PACU) based on fault tree analysis, and to evaluate the predictive performance and relative importance of associated variables using a random forest model.

纳入标准：

1.年龄 ≥18 岁；
2.接受胸外科手术并实施全身麻醉；
3.ASA 分级为 II – IV 级；
4.术后转入 PACU 复苏；
5.围术期核心病历资料、麻醉记录及 PACU 记录完整；

Inclusion criteria

1.Age≥18 years;
2.Patients undergoing thoracic surgery under general anesthesia;
3.American Society of Anesthesiologists physical status class II–IV;
4.Postoperative admission to the post-anesthesia care unit (PACU) for recovery;
5.Complete perioperative core clinical data, including medical history, anesthesia records, and PACU records;

排除标准：

1.术前存在意识障碍，无法进行术后苏醒状态评估者；
2.术前合并严重精神疾病或神经精神障碍者；
3.术中发生严重并发症如心搏骤停等；
4.术后计划性或非计划性转入ICU者；
5.关键研究变量严重缺失者；

Exclusion criteria：

1.Preoperative disturbance of consciousness that precludes postoperative assessment of emergence or recovery status;
2.Severe preoperative psychiatric or neuropsychiatric disorders;
3.Severe intraoperative complications, such as cardiac arrest;
4.Planned or unplanned postoperative transfer to the intensive care unit (ICU);
5.Substantial missingness of key study variables;

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

数据采集形式：本研究采用回顾性病历数据提取方式进行数据采集，研究数据采用统一设计的数据提取表录入，不直接在原始病历中进行标注或修改。数据管理:本研究数据以电子数据在Excel 或WPS表格建立原始数据库；R/Python/SPSS进行数据清洗和统计分析

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

Data will be collected retrospectively from medical records. Relevant preoperative, intraoperative, and post-anesthesia care unit (PACU) recovery-related information will be extracted using a standardized data extraction form. No direct annotation, alteration, or modification will be made to the original medical records. All extracted data will be entered into an electronic database established using Microsoft Excel or WPS spreadsheets. Data cleaning, variable coding, statistical analysis, and model development will be performed using R, Python, and/or SPSS, as appropriate.
