研究目的：

通过体外实验模拟导管内激光碎石，探索不同碎石参数与结石钙化度的关系，寻找最佳的碎石参数，以指导临床操作；对纳入的疑难唾液腺结石病例进行激光碎石治疗，观察碎石效果及术后腺体功能恢复情况，评估碎石术后的中长期疗效；调整优化激光碎石治疗唾液腺结石的技术参数和操作规范，以提高碎石效率和取石成功率。  

Objectives of Study：

To simulate intraductal laser lithotripsy through in vitro experiments, explore the relationship between different lithotripsy parameters and the degree of stone calcification, and identify the optimal lithotripsy parameters to guide clinical practice. To perform laser lithotripsy on enrolled patients with challenging salivary gland calculi, observe the stone fragmentation outcomes and postoperative recovery of glandular function, and evaluate the medium- and long-term efficacy of the lithotripsy procedure. To adjust and optimize the technical parameters and operational protocols of laser lithotripsy for salivary gland calculi, thereby improving stone fragmentation efficiency and the success rate of stone removal.

纳入标准：

体外实验： 1. 形状较规则的类圆形、直径4-6mm、经内镜辅助手术取出的完整唾液腺结石。 钬激光碎石临床病例研究： 1. 唾液腺内镜中心就诊患者，符合下列标准： (1) 4-8mm的嵌顿性腮腺结石，口内途径取石困难，且对口外切口存在明显美学顾虑的患者； (2) 4-8mm的嵌顿性下颌下腺腺门腺内部结石，局麻下口内途径取石困难（患者配合困难或结石难以触及）的患者； (3) 唾液腺内镜检查可直接观察到的结石； (4) 年龄18-70岁。

Inclusion criteria

In vitro experiment: 1. Relatively regular, round or oval-shaped salivary gland stones with a diameter of 4–6 mm, completely removed with the assistance of endoscopic surgery. Clinical case study of holmium laser lithotripsy: 1. Patients attending the Sialendoscopy Center who meet the following criteria: (1) Patients with impacted parotid stones measuring 4–8 mm that are difficult to remove via the intraoral approach and who have significant aesthetic concerns about an external incision; (2) Patients with impacted hilar or intraglandular submandibular stones measuring 4–8 mm that are difficult to remove via the intraoral approach under local anesthesia (due to poor patient cooperation or difficulty in locating the stones); (3) Stones that can be directly visualized during sialendoscopy; (4) Age 18–70 years old.

排除标准：

体外实验： 1. 特殊感染者的唾液腺结石 钬激光碎石临床病例研究： 1. 无症状患者； 2. 因基础性疾病无法耐受手术者； 3. 唾液腺导管口重度狭窄，导管不通畅，无法完成唾液腺内镜检查者。

Exclusion criteria：

In vitro experiment: 1. Patients with salivary gland stones in the presence of specific infections. Clinical case study of holmium laser lithotripsy: 1. Asymptomatic patients; 2. Patients unable to tolerate surgery due to underlying medical conditions; 3. Patients with severe stenosis of the salivary gland duct orifice or ductal obstruction that precludes successful sialendoscopy.
