研究目的:

回顾性分析 2021-2023 年本中心采用腕关节镜下改良经骨隧道联合背侧连续缝合技术修复 TFCC 小凹合并背侧损伤患者的临床资料,明确该改良技术对患者腕关节功能、疼痛程度的改善效果,为临床该类损伤的修复提供针对性循证医学参考依据  

Objectives of Study:

A retrospective analysis was conducted on the clinical data of patients with combined TFCC foveal and dorsal injuries treated in our center from 2021 to 2023 using arthroscopic modified bone tunnel combined with dorsal continuous suture technique. This study aims to clarify the improvement effects of this modified technique on wrist joint function and pain severity, so as to provide targeted evidence?based medical references for the clinical repair of such injuries.

药物成份或治疗方案详述:

 

Description for medicine or protocol of treatment in detail:

 

纳入标准:

1.病史:2021-2023年,入院诊断为TFCC损伤,且保守治疗6周疼痛无好转或用力时疼痛仍明显加重的病例;
2.查体:腕尺侧压痛明显,小凹征阳性,DRUJ背侧压痛,DRUJ推拉试验阳性;
3.影像学:腕关节MR平扫。腕关节冠状位见TFCC 深支在小凹止点处断裂或撕脱,伴或不伴浅支的损伤。矢状位见桡尺背侧韧带肿胀、撕裂、水肿(影像学评估则由两名主治医师分别调阅患者腕关节MR资料后给出结论。将结论一致的病例纳入本次研究,对于结论不一致的病例由主任医师复核。若主任医师认为影像结果符合TFCC深支损伤以及背侧损伤则纳入研究,若不符合则排除);
4.查阅术中DVD资料显示腕关节镜下Hook test阳性;

Inclusion criteria

1.Medical history: Patients diagnosed with TFCC injury between 2021 and 2023, with no pain improvement after 6 weeks of conservative treatment or significant pain exacerbation on exertion.
2.Physical examination: Marked tenderness over the ulnar wrist, positive fovea sign, dorsal tenderness of the DRUJ, and positive DRUJ ballottement test.
3.Imaging: Wrist MRI. Coronal view shows rupture or avulsion of the deep bundle of TFCC at the foveal insertion, with or without superficial bundle injury. Sagittal view shows swelling, tear, or edema of the dorsal radioulnar ligament. (Two attending physicians independently evaluated MRI images; consistent cases were included. Inconsistent cases were reviewed by the chief physician; those meeting the criteria were included, others excluded.);
4.Intraoperative DVD records confirmed a positive arthroscopic Hook test.

排除标准:

1.X线见DRUJ关节炎表现;
2.MR见TFCC中央部穿孔或月骨、尺骨头软骨面下高信号;
3.术中发现TFCC变薄或穿孔或伴有月骨软骨面毛糙、软骨软化等尺骨撞击表现;
4.关节镜术中发现舟月、月三角不稳且需要做韧带修复,包括韧带射频皱缩、缝合修复、克氏针固定;

Exclusion criteria:

1.Radiographic evidence of distal radioulnar joint (DRUJ) arthritis.
2.MRI showing central perforation of the TFCC or subchondral bone marrow edema in the lunate and ulnar head.
3.Intraoperative findings of TFCC thinning or perforation, accompanied by ulnar impaction features such as rough articular cartilage or chondromalacia of the lunate.
4.Arthroscopic identification of scapholunate or lunotriquetral instability requiring ligament repair, including radiofrequency shrinkage, suture repair, or K-wire fixation.
5.Patients with symptomatic pain and discomfort in the contralateral wrist.
