研究目的：

（1）通过混合方法研究，全面评价决策辅助工具在糖尿病慢性创面患者中对改善患者决策质量及临床结局的有效性。 （2）通过质性研究方法，访谈收集使用过该工具的医护人员与患者各方的使用反馈，评价该工具的应用效果、了解应用过程中的阻碍因素，识别未来优化方向。  

Objectives of Study：

(1) Through mixed-methods research, comprehensively evaluate the effectiveness of decision support tools in improving decision quality and clinical outcomes in patients with chronic diabetic wounds. (2) Through qualitative research methods, conduct interviews to collect feedback from healthcare providers and patients who have used the tool, assess the tool's application effectiveness, understand the barriers during its use, and identify directions for future optimization.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1）年龄≥18岁，符合《中国糖尿病防治指南（2024版）》诊断的住院糖尿病患者； （2）创面治疗大于1个月以上未能愈合或无愈合倾向； （3）无精神疾病，有良好的沟通能力； （4）经解释后自愿参加本次研究。

Inclusion criteria

(1) Age >=18 years, hospitalized diabetic patients who meet the diagnostic criteria of the "Chinese Guidelines for the Prevention and Treatment of Diabetes (2024 Edition)"; (2) Wounds that have not healed or show no tendency to heal after more than 1 month of treatment; (3) No mental illness and good communication ability; (4) Voluntary participation in this study after explanation.

排除标准：

（1）肢体已广泛且完全坏死且需即刻做出选择的患者； （2）合并心、脑、肝、肾等主要脏器功能严重损伤的患者； （3）合并有多器官功能衰竭者； （4）沟通能力有障碍的患者； （5）自动出院或放弃治疗者。

Exclusion criteria：

(1) Patients whose limbs are extensively and completely necrotic and require immediate decision-making; (2) Patients with severe damage to major organs such as the heart, brain, liver, and kidneys; (3) Patients with multiple organ failure; (4) Patients with impaired communication abilities; (5) Patients who discharge themselves or abandon treatment.

干预组在常规护理基础上，使用决策辅助工具实施标准化干预。入院后评估决策需求，决策前提供治疗方案信息并澄清价值观，决策时引导共同决策，决策后回顾巩固，出院前给予康复指导与随访计划，全程提升患者决策质量与满意度。

On the basis of routine care, the intervention group used decision support tools to implement standardized interventions. After admission, the decision needs were assessed; before decision-making, information about treatment options was provided and values were clarified; during decision-making, shared decision-making was guided; after decisions, reviews were conducted for consolidation; before discharge, rehabilitation guidance and follow-up plans were provided, enhancing the quality of patient decisions and satisfaction throughout the process.
