研究目的：

基于症状体验模型，综合运用潜在剖面分析、网络分析与混合研究方法，系统揭示血液透析患者症状群的异质性特征与内部互动机制，并构建“医院-社区-家庭”分级联动症状管理模式，为精准化症状管理提供实证依据与实践方案。  

Objectives of Study：

Based on the Symptom Experience Model, this study comprehensively employs latent profile analysis, network analysis, and mixed methods research to systematically reveal the heterogeneous characteristics and internal interaction mechanisms of symptom clusters in hemodialysis patients. Furthermore, it develops a tiered collaborative "hospital-community-family" symptom management model, providing empirical evidence and practical approaches for precision symptom management.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1. 符合国际肾脏病协会 ESRD 诊断标准，以血液透析为肾脏替代治疗方式； 2. 年龄 >= 18 周岁； 3. 意识清楚，无认知障碍，能正常交流； 4. 能自主使用智能手机等设备； 5. 签署知情同意书并自愿参加。

Inclusion criteria

1. Meets the ESRD diagnostic criteria of the International Society of Nephrology, with hemodialysis as the renal replacement therapy; 2. Age >= 18 years old; 3. Conscious, without cognitive impairment, able to communicate normally; 4. Able to use smartphones and other devices independently; 5. Signs the informed consent form and voluntarily participates.

排除标准：

1.近期行重大手术者； 2.合并肾脏病以外严重恶性疾病者；

Exclusion criteria：

1.Individuals who have undergone major surgery in the recent past; 2.Severe malignant diseases unrelated to kidney disease;
