研究目的：

本项单中心前瞻性队列研究旨在评估LV小尺寸是否能独立预测24个月随访期间的AF负荷进展。通过重复心律监测可评估负荷轨迹，事件发生时间模型将用于评估阈值跨越事件。此外，嵌套病例对照设计可对风险集内进展时间点的暴露对比进行详细评估，在保持时间效度的同时提高分析效率。通过整合结构表型与纵向负荷评估，本研究旨在阐明心室几何形态对AF疾病演变的贡献。  

Objectives of Study：

This single-center prospective cohort study aims to evaluate whether a small left ventricular (LV) size can independently predict the progression of atrial fibrillation (AF) burden during a 24-month follow-up period. The burden trajectory can be evaluated through repeated rhythm monitoring, and an event time model will be used to assess threshold-crossing events. In addition, a nested case-control design can be used to conduct a detailed assessment of the exposure contrast at the time points of progression within the risk set, improving the analysis efficiency while maintaining temporal validity. By integrating structural phenotypes and longitudinal burden assessment, this study aims to elucidate the contribution of ventricular geometry to the evolution of AF.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1. 年龄>=18岁； 2. 入组前6个月内经心电图证实存在心房颤动（通过12导联心电图、动态监测或设备诊断确认，符合既定AF队列研究方法学； 3. 可获得足够质量的基线超声心动图图像以进行定量左心室评估； 4. 能够接受结构化纵向AF负荷监测； 5. 提供书面知情同意。

Inclusion criteria

1. Age >=18 years; 2. Documented atrial fibrillation confirmed by electrocardiogram within 6 months before enrollment (confirmed by 12-lead ECG, Holter monitoring, or device diagnosis, in accordance with the established AF cohort study methodology); 3. Availability of baseline echocardiographic images of sufficient quality to allow quantitative left ventricular assessment; 4. Ability to undergo structured longitudinal AF burden monitoring; 5. Provision of written informed consent.

排除标准：

1. 显著影响心室形态的结构性心肌病，包括肥厚型心肌病、浸润性心肌病或具有血流动力学意义的先天性心脏病；2. 入组前3个月内有急性冠状动脉综合征、心脏手术或失代偿性心力衰竭病史； 3. 永久性心室起搏依赖或心脏再同步化治疗导致无法准确评估心房节律； 4. 因非心脏疾病导致预期寿命不足1年或预计无法完成随访。

Exclusion criteria：

1. Structural cardiomyopathies that significantly affect ventricular morphology, including hypertrophic cardiomyopathy, infiltrative cardiomyopathy, or congenital heart disease with hemodynamic significance; 2. History of acute coronary syndrome, cardiac surgery, or decompensated heart failure within 3 months prior to enrollment; 3. Permanent ventricular pacing dependence or cardiac resynchronization therapy preventing accurate assessment of atrial rhythm; 4. Non-cardiac conditions resulting in an expected life expectancy of less than 1 year or inability to complete follow-up.
