研究目的：

比较肱肌平面阻滞和腋路臂丛神经阻滞对桡骨远端骨折手术患者术后的镇痛效果，以及比较术后患者肩、肘关节和小指的肌肉活动度  

Objectives of Study：

Compare the postoperative analgesic effects of the brachial plane block and axillary brachial plexus block in patients undergoing distal radial fracture surgery, and compare the postoperative muscle activity of the shoulder, elbow, and little finger in patients.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1. 18岁-60岁患者， 2.ASA I-III级， 3.拟行单侧桡骨远端骨折手术喉罩全麻手术。

Inclusion criteria

1. Patients aged 18-60 years, 2. ASA I-III, 3. Scheduled for unilateral distal radius fracture surgery under laryngeal mask general anesthesia.

排除标准：

1.穿刺部位感染。 2.前臂手术史。 3.已有上肢神经损伤。 4.局部麻醉禁忌症。 5.局部麻醉药过敏史。 6.凝血功能障碍。 7.药物或酒精滥用。 8.每日服用止痛药。 9.合并其他部位外伤。 10.术前谵妄、帕金森病、痴呆症、严重神经系统疾病、3个月内卒中、重度肝功能不全（Child-Pugh C级）、肾功能衰竭。

Exclusion criteria：

1. Infection at the puncture site. 2. History of forearm surgery. 3. Existing upper limb nerve injury. 4. Contraindications to local anesthesia. 5. History of allergy to local anesthetics. 6. Coagulation disorders. 7. Drug or alcohol abuse. 8. Daily use of painkillers. 9. Combined trauma to other sites. 10. Preoperative delirium, Parkinson's disease, dementia, severe neurological disorders, stroke within the past 3 months, severe liver dysfunction (Child-Pugh class C), renal failure.

术前30min，抽取局麻药0.5%罗哌卡因25ml，在肘横纹上方寻找三股神经正中神经（MN），桡神经（RN）和前臂外侧皮神经（LCNF），在肱肌表面筋膜推注25ml 0.5%浓度的罗哌卡因进行神经阻滞。

Thirty minutes before surgery, 25 ml of 0.5% ropivacaine was drawn for local anesthesia. Above the elbow crease, the three nerves—the median nerve (MN), radial nerve (RN), and lateral cutaneous nerve of the forearm (LCNF)—were located. Then, 25 ml of 0.5% ropivacaine was injected on the fascia surface of the biceps muscle for nerve block.

Intervention：

30 minutes before surgery, 25 ml of 0.5% ropivacaine was drawn for local anesthesia. The median nerve (MN) was located lateral to the axillary artery in the armpit, the radial nerve (RN) was located below the medial side of the axillary artery, the ulnar nerve (UN) was located under the medial superficial fascia, and the musculocutaneous nerve, forming a triangular or diamond-shaped mesh between the biceps brachii and coracobrachialis muscles. About 6 ml of 0.5% ropivacaine was injected to block each nerve.
