研究目的：

本研究旨在构建心脏外科围手术期营养管理方案，并通过临床实践进行有效性检验，为改善心脏外科患者围手术期营养状况以及临床预后结局提供新思路及参考依据。  

Objectives of Study：

This study aims to establish a perioperative nutritional management protocol for cardiac surgery and validate its efficacy through clinical practice, thereby providing novel insights and reference guidelines for improving the perioperative nutritional status and clinical outcomes of cardiac surgery patients.

药物成份或治疗方案详述：

1.基于 NCPM 的心脏外科患者围手术期营养管理方案构建 （1）证据总结：系统检索心脏外科围术期营养管理文献、指南及 NCPM 应用案例， 运用 GRADE工具分级筛选，RCT 研究、临床指南及 Meta 分析，聚焦营养筛查工具、评估指标、干预策略以及检测指标证据，构建 NCPM 框架下的循证依据。 （2）方案构建：基于 NCPM 模型整合循证证据结果，通过多学科协作构建包含评估、诊断、干预和评价四大核心模块的围术期营养管理方案，经专家会议和预实验验证后形成标准化操作体系。 2.基于 NCPM 的心脏外科患者围手术期营养管理方案实证研究 采用类试验研究，比较分析干预组（基于 NCPM 的心脏外科病患者围手术期营养管理方案）与对照组（常规营养管理）在干预前和干预后的营养相关指征－人体测量学指标（BMI 、体重、握力、上臂围、肱三头肌皮褶厚度、腰围、小腿围）、营养生化指标（总蛋白、白蛋白、前蛋白、血红蛋白）、6 分钟步行试验、生活质量、并发症发生率和住院时间的变化。 

Description for medicine or protocol of treatment in detail：

1.Development of a Perioperative Nutrition Management Protocol for Cardiac Surgery Patients Based on NCPM (1) Evidence Summary: A systematic review was conducted of literature, guidelines, and NCPM application cases on perioperative nutrition management in cardiac surgery. Using the GRADE tool for classification screening, RCT studies, clinical guidelines, and meta-analyses were analyzed to identify evidence regarding nutritional screening tools, assessment indicators, intervention strategies, and monitoring metrics, thereby establishing an evidence-based framework under the NCPM paradigm. (2) Protocol Development: Based on the NCPM model and integrated evidence-based findings, a perioperative nutrition management protocol was developed through multidisciplinary collaboration, encompassing four core modules: assessment, diagnosis, intervention, and evaluation. This protocol was standardized following expert review and preclinical validation. 2.An Empirical Study on the Perioperative Nutrition Management Protocol for Cardiac Surgery Patients Based on NCPM: This quasi-trial study compared and analyzed the intervention group (perioperative nutrition management protocol based on NCPM for cardiac surgery patients) with the control group (conventional nutrition management) regarding nutritional-related indicators—anthropometric parameters (BMI, body weight, grip strength, upper arm circumference, triceps skinfold thickness, waist circumference, calf circumference)—nutritional biochemical indicators (total protein, albumin, prealbumin, hemoglobin), 6-minute walk test, quality of life, complication incidence, and changes in hospital stay before and after intervention. 

纳入标准：

1. 年龄>=18周岁； 2. 符合冠心病、心脏瓣膜病诊断标准[葛均波]，首次接受择期开胸心脏直视手术的患者：冠状动脉旁路移植术、心脏瓣膜手术（二尖瓣置换、主动脉瓣置换、双瓣膜置换、Bentall、三尖瓣置换、二尖瓣成形、三尖瓣成形）、冠状动脉旁路移植术合并任一上述瓣膜手术的联合手术； 3. 认知正常，能进行有效沟通交流者； 4. 知情同意参与本研究。

Inclusion criteria

1. Age >=18 years; 2. Patients meeting the diagnostic criteria for coronary artery disease (CAD) or cardiac valve disease [Ge Junbo], who are undergoing elective open-heart surgery for the first time: coronary artery bypass grafting (CABG), cardiac valve surgery (mitral valve replacement, aortic valve replacement, double-valve replacement, Bentall procedure, tricuspid valve replacement, mitral valve repair, tricuspid valve repair), or CABG combined with any of the aforementioned valve surgeries; 3. Normal cognitive function and ability to communicate effectively; 4. Informed consent to participate in this study.

排除标准：

1. 存在躯体功能障碍者； 2. 严重肝功能障碍、肾功能不全（术前长期肾脏替代治疗）、胃肠道手术及功能障碍、未控制的甲状腺功能亢进或减退； 3. 恶性肿瘤的患者； 4. 存在心脏以外其他器官衰竭的患者。

Exclusion criteria：

1. Patients with somatic dysfunction; 2. Severe hepatic dysfunction, renal insufficiency (with long-term renal replacement therapy prior to surgery), gastrointestinal surgery or dysfunction, uncontrolled hyperthyroidism or hypothyroidism; 3. Patients with malignant tumors; 4. Patients with failure of organs other than the heart.
