本项目面向退行性颈脊髓病,依托北京市多家头部医院协力,将建立专病数据库,整合电子病历、影像、实验室与量表、可穿戴数据等多模态信息,构建“评估—预测—干预—随访”的闭环诊疗体系。将建立≥2000例专病数据库,研发多模态深度学习与因果推断融合的手术疗效与并发症预测模型,并在知识图谱与检索增强基础上形成可对话、可追溯、可审计的专病智能体,嵌入门诊—病房—康复—社区的临床流程。通过4家三甲医院开展≥700例、随访≥12个月的多中心前瞻性研究,验证模型与智能体的有效性、安全性与可推广性;同步制定“北京市版”智能化临床路径、应用与数据合规规范。  

Objectives of Study:

This project focuses on degenerative cervical myelopathy (DCM). With the efforts of multiple leading hospitals in Beijing, we will establish a disease-specific database that integrates multi-modal information, including electronic medical records, imaging, laboratory results, scales, and wearable device data, to build a diagnostic and treatment system.Specifically, we will: 1.Establish a disease-specific database with ≥2,000 cases. 2.Develop predictive models for surgical outcomes and complications by integrating multi-modal deep learning with causal inference. 3.Build a disease-specific, conversational, traceable, and auditable intelligent agent based on knowledge graphs and retrieval-augmented generation (RAG), embedding it into the clinical workflow through outpatient clinics, wards, rehabilitation, and community care. 4.Conduct a multi-center prospective study across four hospitals involving ≥700 cases with ≥12 months of follow-up to validate the effectiveness, safety, and generalizability of the models and the intelligent agent. 5.Simultaneously establish a "Beijing version" of intelligent clinical pathways, as well as application and data compliance standards.

纳入标准:

回顾性数据库建设(研究一) 1. 年龄≥18 岁; 2. 符合上述 DCM 诊断标准; 3. 于 2005 年 1 月 1 日至 2025 年 12 月 31 日期间,在任一合作 中心就诊并留存可抽取的临床/影像数据。 前瞻性临床验证(研究二) 1. 年龄>=18 岁; 2. 经临床和影像学检查明确诊断为 DCM; 3. 能够理解并签署针对本研究的特定知情同意书; 4. 能够完成至少 12 个月的随访。

Inclusion criteria

Retrospective Database Construction (Study 1) 1. Age >= 18 years; 2. Meet the above DCM diagnostic criteria; 3. Visited any participating center between January 1, 2005, and December 31, 2025, with extractable clinical/imaging data available. Prospective Clinical Validation (Study 2) 1. Age >= 18 years; 2. Clinically and radiologically confirmed diagnosis of DCM; 3. Able to understand and sign the study-specific informed consent for this research; 4. Able to complete at least 12 months of follow-up.

排除标准:

回顾性数据库建设(研究一) 1. 外伤、肿瘤、感染、炎症或先天畸形等非退行性脊髓病变; 2. 既往颈椎手术史; 3. 严重认知障碍/精神疾病,无法完成评估者; 4. 关键变量缺失且多重插补后仍不可用者(按预设质控规则判定) 前瞻性临床验证(研究二) 1. 由外伤、肿瘤、感染、先天畸形等非退行性因素导致的颈脊髓受压; 2. 既往有颈椎手术史; 3. 合并严重的全身系统疾病或预计生存期小于 2 年; 4. 依从性差,无法配合完成随访。

Exclusion criteria:

Retrospective Database Construction (Study One) 1. Non-degenerative spinal cord lesions such as trauma, tumors, infection, inflammation, or congenital malformations; 2. History of previous cervical spine surgery; 3. Severe cognitive impairment/mental illness preventing completion of assessments; 4. Key variables missing and still unusable after multiple imputation (determined according to preset quality control rules) Prospective Clinical Validation (Study Two) 1. Cervical spinal cord compression caused by non-degenerative factors such as trauma, tumors, infection, or congenital malformations; 2. History of previous cervical spine surgery; 3. Presence of severe systemic diseases or expected survival less than 2 years; 4. Poor compliance, unable to cooperate with follow-up.
