研究目的:

主要目的: 1. 前瞻性验证 P&P-CR 模型 AKI 的区分能力(AUC)。 2. 验证模型预测概率与实际观测概率的校准度(校准斜率、平均绝对误差 MAE)。 次要目的: 1. 更新 AKI 风险分层最优截断值(Youden 指数最大化)。 2. 绘制决策曲线(DCA),量化阈值 5–50% 范围内的临床净获益。 3. 开发、上线并持续更新维护开源预测模型程序。  

Objectives of Study:

Primary objectives: 1. Prospectively validate the discriminative ability (AUC) of the P&P-CR model for AKI. 2. Validate the calibration of the model's predicted probabilities with actual observed probabilities (calibration slope, mean absolute error MAE). Secondary objectives: 1. Update the optimal cut-off values for AKI risk stratification (maximizing the Youden index). 2. Plot decision curves (DCA) to quantify the clinical net benefit within the threshold range of 5–50%. 3. Develop, launch, and continuously maintain and update the open-source prediction model program.

药物成份或治疗方案详述:

 

Description for medicine or protocol of treatment in detail:

 

纳入标准:

1. 年龄 18-75 岁; 2. 组织学确诊上皮性卵巢癌/输卵管癌/原发性腹膜癌; 3. FIGO III-IV 期; 4. 接受顺铂 HIPEC(剂量 75 mg/m^2,温度 43 °C,灌注时间 90 min)。

Inclusion criteria

1. Age 18-75 years; 2. Histologically confirmed epithelial ovarian/fallopian tube/primary peritoneal cancer; 3. FIGO stage III-IV; 4. Undergoing cisplatin HIPEC (75 mg/m^2, 43°C, 90 min).

排除标准:

1. HIPEC 前已存在肾功能不全状态; 2. 同期使用其他肾毒性药物(如万古霉素、两性霉素 B)> 24 h 或正使用其他肾脏保护药物; 3. 既往接受过顺铂 HIPEC; 4. 一般状态不佳,经评估无法完成随访或依从性不好。

Exclusion criteria:

1. Pre-existing renal dysfunction before HIPEC; 2. Concurrent use of other nephrotoxic drugs (e.g., vancomycin, amphotericin B) > 24 h or use of other renal protective agents; 3. Previous cisplatin HIPEC; 4. Poor general condition, unable to complete follow-up or poor compliance.
