研究目的：

1. 比较两组术后不同时间点（如1、3个月）嗅觉功能及鼻腔相关生活质量评分，分析改良术式对嗅觉及鼻腔功能保护的优势。 2. 比较两组鼻腔并发症（鼻腔大出血、鼻中隔穿孔、鼻腔黏连、慢性鼻窦炎及蝶窦黏膜囊肿等）的发生率。 3. 比较两种术式在肿瘤切除率及内分泌功能改善、手术时间、术中出血量及住院时长等方面的差异，确保改良术式在肿瘤控制方面不劣于传统术式。  

Objectives of Study：

1. To compare olfactory function and nasal quality of life scores between the two groups at different postoperative time points (e.g., 1 and 3 months), and analyze the advantages of the modified surgical approach in protecting olfactory and nasal function. 2. To compare the incidence rates of nasal complications between the two groups, including major nasal hemorrhage, nasal septum perforation, nasal adhesion, chronic sinusitis, sphenoid sinus mucosal cyst, etc. 3. To compare differences between the two surgical procedures in terms of tumor resection rate, improvement of endocrine function, operation time, intraoperative blood loss, and length of hospital stay, so as to ensure that the modified procedure is non-inferior to the conventional one in tumor control.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1. 年龄 >=18 岁； 2. 经术前 MRI 和术后病理学确诊为垂体腺瘤，适合经鼻蝶窦入路手术治疗； 3. 首次手术，无既往经鼻颅底手术或蝶窦手术史； 4. 术前鼻内镜及鼻窦 CT 显示蝶窦发育良好、蝶窦口可辨，无严重鼻窦炎； 5. 能配合完成嗅觉测试与随访评估，签署知情同意。

Inclusion criteria

1. Age >= 18 years; 2. Pituitary adenoma confirmed by preoperative MRI and postoperative pathology, suitable for transsphenoidal surgery; 3. First operation, no history of previous transnasal skull base or sphenoid sinus surgery; 4. Preoperative nasal endoscopy and paranasal sinus CT showing well-developed sphenoid sinus, identifiable sphenoid ostium, and no severe sinusitis; 5. Able to cooperate with olfactory testing and follow-up assessments, and signed informed consent.

排除标准：

1. 需行明显扩大经鼻入路（广泛鞍旁、鞍上或额底肿瘤切除）且预计存在高流量脑脊液（CSF）漏者； 2. 鼻腔解剖严重异常（重度鼻中隔偏曲、多发鼻息肉、霉菌性鼻窦炎等）； 3. 术前既有中重度嗅觉障碍或严重鼻腔疾病； 4. 合并严重全身疾病无法耐受手术或长期随访者。

Exclusion criteria：

1. Patients requiring significantly extended endonasal approach for resection of extensive parasellar, suprasellar or frontal basal tumors with anticipated high-flow cerebrospinal fluid (CSF) leakage; 2. Severe anatomical abnormalities of the nasal cavity (severe nasal septum deviation, multiple nasal polyps, fungal sinusitis, etc.); 3. Preoperative moderate to severe olfactory dysfunction or severe nasal diseases; 4. Complicated with severe systemic diseases intolerant to surgery or long-term follow-up.

Blinding：

Due to the nature of surgical procedures, blinding of surgeons and patients is not feasible. To minimize subjective bias, single blinding was applied to outcome assessors, who were independent investigators not involved in surgical procedures and remained unaware of group allocation during assessment.
