研究目的：

通过动态姿势管理干预试验，阐明颅骶节律重调与吞咽功能改善的时程效应关系，验证姿势调整对颅骶系统生物力学平衡的干预效果，为中枢性吞咽功能障碍提供以姿势-颅骶协同调控为核心的康复治疗新路径。 基于表面肌电图sEMG分析与姿势特性量化，系统解析脑卒中后吞咽功能障碍患者姿势肌群募集时序紊乱、强度失衡及动作模式代偿特征，识别与吞咽功能高度相关的核心姿势肌群，明确姿势控制与吞咽动作的神经肌肉耦联规律。 通过手法评估与影像评估手段，明确脑卒中后吞咽功能障碍患者颅骶节律异常特征及颅骨位置偏移模式，结合吞咽中枢功能区的fMRI激活特征分析，揭示颅骨生物力学结构改变与吞咽功能障碍的中枢调控关联机制。  

Objectives of Study：

Through dynamic posture management intervention experiments, the temporal effect relationship between craniosacral rhythm readjustment and swallowing function improvement was elucidated, and the intervention effect of posture adjustment on the biomechanical balance of the craniosacral system was verified. This provides a new rehabilitation treatment path for central swallowing dysfunction with posture craniosacral coordinated regulation as the core. Based on surface electromyography (sEMG) analysis and posture characteristic quantification, the system analyzes the temporal disorder, intensity imbalance, and compensatory characteristics of posture muscle group recruitment in post-stroke swallowing dysfunction patients, identifies core posture muscle groups highly related to swallowing function, and clarifies the neuromuscular coupling law between posture control and swallowing movements. By using manual and imaging evaluation methods, the abnormal characteristics of craniosacral rhythms and skull displacement patterns in patients with post-stroke swallowing dysfunction were identified. Combined with the analysis of fMRI activation characteristics of swallowing central functional areas, the central regulatory mechanism of the relationship between changes in skull biomechanical structure and swallowing dysfunction was revealed.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.符合全国第6次脑血管病学术会议制订的脑卒中诊断标准； 2.年龄18～80岁； 3.患者意识清楚，生命体征稳定，可以配合完成评定与治疗； 4.具有吞咽功能障碍，洼田饮水试验为Ⅱ～Ⅴ级； 5.患者知情同意，并签署知情同意书。

Inclusion criteria

1. Meets the diagnostic criteria for stroke formulated at the 6th National Conference on Cerebrovascular Disease; 2. Age range: 18-80 years old; 3. The patient has clear consciousness, stable vital signs, and can cooperate to complete the assessment and treatment; 4. Having swallowing dysfunction, the Wada drinking water test is rated as level II-V; 5. The patient provides informed consent and signs the informed consent form.

排除标准：

1.颅内高压（如脑水肿、脑出血）、颅骨骨折的患者； 2.去骨瓣减压的患者； 3.凝血功能障碍的患者； 4.重度认知功能障碍得的患者； 5.合并心血管、肝、肾、消化系统等严重原发性疾病； 6.既往有脊髓损伤或脊髓相关疾病的患者； 7.体内带有磁性金属植入物的患者； 8.妊娠期的患者； 9.在治疗或评估时无法保持静止或配合的患者。

Exclusion criteria：

1. Patients with intracranial hypertension (such as cerebral edema, cerebral hemorrhage) and skull fractures; 2. Patients undergoing bone flap decompression; 3. Patients with coagulation dysfunction; 4. Patients with severe cognitive impairment; 5. Merge serious primary diseases such as cardiovascular, liver, kidney, digestive system, etc; 6. Patients with previous spinal cord injuries or spinal cord related diseases; 7. Patients with magnetic metal implants in their bodies; 8. Pregnant patients; 9. Patients who are unable to remain still or cooperate during treatment or evaluation.
