研究目的：

探索老年慢性心力衰竭患者症状群随时间的变化轨迹，并明确其核心症状。  

Objectives of Study：

To explore the trajectory of symptom clusters over time in elderly patients with chronic heart failure, and to identify the core symptoms.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1. 被诊断为慢性心力衰竭； 2. 年龄为 65-90 周岁； 3. 具有正常沟通交流能力； 4. 纽约心功能分级（NYHA）为 II-IV 级（因心功能 I 级的患者日常体力活动不引起明显的呼吸困难、疲乏等症状，故不纳入）。

Inclusion criteria

1. Diagnosed with chronic heart failure; 2. Aged 65-90 years; 3. Having normal communication abilities; 4. New York Heart Association (NYHA) functional class II-IV (patients with class I heart function are not included because their daily physical activities do not cause obvious symptoms such as shortness of breath or fatigue).

排除标准：

1. 合并恶性肿瘤或重要器官功能障碍； 2. 既往或目前有精神心理疾病； 3. 心脏移植术后； 4. 近期遭遇重大生活事件（如离婚、丧偶、亲人离世等）。

Exclusion criteria：

1. Combined malignant tumors or dysfunction of important organs; 2. Past or current mental or psychological disorders; 3. After heart transplantation; 4. Recently experienced major life events (such as divorce, widowhood, or the death of a loved one).

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

本研究采用病例记录表（CRF）进行纸质数据采集。数据录入采用EpiData 3.1，随后导入SPSS 26.0进行逻辑核查与清理。所有纸质CRF存放于带锁档案柜，电子数据存放于课题组专用加密计算机，仅课题组核心成员可访问。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

Data were collected using a paper-based Case Record Form with double-checking by two independent researchers. Data entry was performed using EpiData 3.1, followed by logical verification and cleaning in SPSS 26.0. All paper CRFs are stored in a locked filing cabinet, and electronic data are stored on an encrypted, password-protected computer accessible only to core research team members.
