研究目的：

采用前期构建的精准自我管理方案对老年高血压合并衰弱患者进行干预，探讨其在老年高血压合并衰弱患者的衰弱状况、血压水平、自我管理水平和生活质量等方面的应用效果。  

Objectives of Study：

This study adopted the previously constructed precise self-management program to intervene in elderly hypertensive patients with frailty, and explored its application effects on frailty status, blood pressure level, self-management ability and quality of life among elderly hypertensive patients with frailty.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1. 符合原发性高血压的诊断标准； 2. 合并衰弱（采用 Fried 衰弱表型量表进行评估；评分>=1 分）； 3. 年龄>=60 岁； 4. 理解能力正常，表达和交流能力无异常； 5. 能够掌握电子手环的使用方法； 6. 对本研究知情同意。

Inclusion criteria

1. Meet the diagnostic criteria for essential hypertension; 2. Complicated with frailty (assessed by the Fried Frailty Phenotype Scale; score >= 1 points); 3. Aged >= 60 years old; 4. Normal cognitive comprehension ability with unimpaired expression and communication skills; 5. Able to master the usage of electronic wristbands; 6. Voluntarily sign informed consent for this study.

排除标准：

1. 处于慢性病急性期或存在重大并发症的患者； 2. 肢体功能存在病变影响运动的患者； 3. 被明确诊断为精神类疾病或存在认知障碍； 4. 同时参加其他研究者。

Exclusion criteria：

1. Patients in the acute phase of chronic diseases or with severe complications; 2. Patients with limb dysfunction affecting physical activity; 3. Patients diagnosed with mental disorders or cognitive impairment; 4. Those participating in other clinical research simultaneously.
