研究目的：

1.分析老年慢性心衰患者的社会衰弱现状及其与抑郁、认知功能的关系，通过资料评估老年慢性心衰患者社会衰弱的状况，并系统分析其与认知功能及抑郁症状之间的关系，探索社会衰弱、认知功能和抑郁之间的潜在交互关系； 2.绘制老年慢性心衰患者短期内社会衰弱的发展轨迹并探究其影响因素，追踪老年慢性心衰患者在住院期间、出院后三个月及六个月的社会衰弱发展轨迹，进一步识别影响社会衰弱变化的关键因素，为制定个性化干预措施提供数据支持和实践指导  

Objectives of Study：

1. Analyze the current situation of social frailty in elderly patients with chronic heart failure and its relationship with depression and cognitive function. Evaluate the status of social frailty in elderly patients with chronic heart failure through data, and systematically analyze its relationship with cognitive function and depressive symptoms. Explore the potential interactive relationships among social frailty, cognitive function, and depression. 2. Draw the development trajectory of social frailty in elderly patients with chronic heart failure in the short term and explore its influencing factors. Track the development trajectory of social frailty in elderly patients with chronic heart failure during hospitalization, three months after discharge, and six months after discharge. Further identify the key factors influencing the change in social frailty, and provide data support and practical guidance for formulating personalized intervention measures.

纳入标准：

1.年龄>=60岁； 2.符合《中国心力衰竭诊断和治疗指南2024》慢性心力衰竭的诊断标准； 3.美国纽约心脏病学会（New York Heart Association, NYHA）心功能分级Ⅱ-Ⅳ级； 4.无理解和阅读障碍； 5.自愿加入本次研究者

Inclusion criteria

1. Age >= 60 years old; 2. Meets the diagnostic criteria for chronic heart failure as stipulated in the "2024 Chinese Guidelines for Diagnosis and Treatment of Heart Failure"; 3. NYHA cardiac function classification is at level II-IV; 4. Has no comprehension or reading disabilities; 5. Voluntarily participates in this study.

排除标准：

1.伴随其他严重疾病、恶性肿瘤、终末期、透析患者； 2.听力障碍或语言交流困难者； 3.经过临床诊断患者被确诊为阿尔茨海默病或属于其他类型的痴呆症者； 4.既往有抑郁、焦虑或其他精神障碍史

Exclusion criteria：

1. Along with other serious diseases, malignant tumors, patients in terminal stage, and those undergoing dialysis; 2. Those with hearing impairment or difficulty in language communication; 3. Patients diagnosed clinically with Alzheimer's disease or other types of dementia; 4. Those with a history of depression, anxiety or other mental disorders in the past.
