研究目的：

通过开展“米力农联合每搏量变异度指导控制性低中心静脉压技术在肝脏切除术中对重要脏器血流灌注的影响”项目，研究该技术对于术中脑、肾血流灌注及术后神经认知功能发生率及程度的影响  

Objectives of Study：

Through the project "Effect of milrinone combined with stroke volume variation-guided controlled low central venous pressure technique on perfusion of vital organs during hepatectomy", the effects of this technique on intraoperative cerebral and renal perfusion as well as the incidence and severity of postoperative neurocognitive dysfunction were studied.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

纳入2026.05-2027.05我院全麻下择期行全身麻醉下腹腔镜肝部分切除术患者；性别不限；年龄：18-80岁；BMI:18-30kg/m2；ASA分级：I-Ⅲ级；获得患者知情同意。

Inclusion criteria

Patients who underwent elective laparoscopic partial hepatectomy under general anesthesia at our hospital from May 2026 to May 2027 were included. Neither sex was restricted. Age: 18–80 years. BMI: 18–30 kg/m2. ASA classification: I–III. Informed consent was obtained from the patients.

排除标准：

术前存在严重心脑血管疾病史；心律失常；重要脏器功能严重不全；严重低血压；休克；长期服用镇静镇痛药；拒绝签署知情同意书。

Exclusion criteria：

Preoperative history of severe cardiovascular or cerebrovascular disease; arrhythmia; severe dysfunction of vital organs; severe hypotension; shock; long-term use of sedatives and analgesics; refusal to sign informed consent form.

Based on the guidance of permissive high SVV (13%-18%), restrictive fluid infusion is performed. When SVV 18%, appropriate trial fluid infusion is conducted. At the start of surgery, milrinone is continuously infused at a rate of 0.2-0.5 μg/kg/min to maintain cardiac index (CI) > 2.5 L/min/m^2.
