研究目的：

本研究拟在行单侧全膝关节置换术老年患者中，评价术中亚麻醉剂量艾司氯胺酮持续输注对术后睡眠障碍的预防效果及安全性，为老年大关节手术围手术期睡眠管理提供新的循证依据与精准用药方案。  

Objectives of Study：

This study aims to evaluate the preventive effect and safety of continuous intraoperative infusion of a sub-anesthetic dose of esketamine on postoperative sleep disturbance in elderly patients undergoing unilateral total knee arthroplasty, and to provide new evidence-based support and a precise medication regimen for perioperative sleep management in elderly patients undergoing major joint surgery.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.年龄60周岁-74周岁，性别不限 2.择期全身麻醉下行单侧全膝关节置换术的患者 3.ASA I～II级 4.18.5kg/m2 ≤ BMI < 32.5kg/m2

Inclusion criteria

1.Age range: 60 to 74 years. Both sexes are included. 2.Patients undergoing elective unilateral total knee arthroplasty under general anesthesia. 3.American Society of Anesthesiologists (ASA) physical status classification I-II. 4.18.5kg/m2 ≤ BMI < 32.5kg/m2.

排除标准：

1.拒绝参加的患者 2.对麻醉药品过敏 3.既往恶性高热病史或家族史 4.局麻药过敏史，穿刺部位局部皮肤感染、出血等 5.凝血功能严重异常 6.严重心、肺、肝、肾功能障碍 7.严重精神障碍 8.有药物成瘾或滥用史

Exclusion criteria：

1.Patients who decline to participate. 2.Allergic to anesthetics. 3.History or family history of malignant hyperthermia. 4.History of local anesthetic allergy, local skin infection, bleeding at puncture site, etc. 5.Severe coagulation dysfunction. 6.Severe cardiac, pulmonary, hepatic, or renal dysfunction. 7.Severe mental disorders. 8.History of drug addiction or abuse.
