研究目的：

（1）构建贵州省农村地区 PICS 居家患者远程精准健康管理模式 （2）验证 PICS 居家患者远程精准健康管理模式的适用性和有效性，并探索项目实施过程中的促进和障碍因素。  

Objectives of Study：

(1) Establish a remote and precise health management model for PICS patients in rural areas of Guizhou Province. (2) Verify the applicability and effectiveness of the remote and precise health management model for PICS patients at home, and explore the facilitating and obstructive factors during the project implementation process.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.年龄≥18 岁； 2.诊断为PICS； 3.知晓病情； 4.能独立阅读和写作； 5.居住在农村； 6.至少有一部智能手机并可熟练地进行操作该手机； 7.愿意配合使用远程健康管理方案进行自我管理； 8.自愿参加本研究。

Inclusion criteria

1. Age >= 18 years old; 2. Diagnosed with PICS; 3. Aware of the condition; 4. Capable of independent reading and writing; 5. Residing in rural areas; 6. Have at least one smart phone and be proficient in operating it; 7. Willing to cooperate with the use of remote health management programs for self-management; 8. Voluntarily participate in this study.

排除标准：

1.生活不能自理或不合作或伴有精神障碍； 2.病情不稳定，有生命危险或临终患者。

Exclusion criteria：

1. Unable to take care of oneself or uncooperative, or with mental disorders; 2. Unstable condition, in critical condition or a terminally ill patient.

研究正式开始前，由一名不参与本研究的第三方人员使用随机化网站生成器（https://www.randomizer.org/）产生随机数字序列，按照 1:1 的比例将患者分为干预组和对照组，保证两组患者人数均等。

Before the formal study began, a third-party person who was not involved in this study used a randomization website generator (https://www.randomizer.org/) to generate a random sequence of numbers. The patients were then divided into the intervention group and the control group in a 1:1 ratio to ensure that the number of patients in both groups was equal.
