研究目的：

通过文献回顾和专家函询构建帕金森病患者居家跌倒风险护理清单，并通过前瞻性单组前后对照研究评价该清单在临床应用中的可行性及初步效果。观察干预前后患者跌倒恐惧、自我效能、平衡及生活质量等变化。  

Objectives of Study：

A care checklist for assessing the risk of falls at home for individuals with Parkinson’s disease was developed through a literature review and expert consultations. The feasibility and initial effects of this checklist in clinical practice were evaluated through a prospective, single-group, before-and-after controlled study. Changes in patients’ fear of falling, self-efficacy, balance, and quality of life were observed before and after the intervention.

纳入标准：

1.患者纳入标准： (1)经临床诊断为帕金森病； (2)年龄>18岁； (3)意识清楚，能配合问卷与随访； (4)自愿签署知情同意书。 2.照护者纳入标准： (1)能正常沟通并完成问卷; (2)自愿参加并签署知情同意。

Inclusion criteria

1. Patient Inclusion Criteria: (1) Clinically diagnosed with Parkinson's disease; (2) Age > 18 years; (3) Conscious and able to cooperate with questionnaires and follow-up; (4) Voluntarily signs the informed consent form. 2. Caregiver Inclusion Criteria: (1) Able to communicate normally and complete the questionnaire; (2) Voluntarily participates and signs the informed consent form.

排除标准：

1.患者排除标准： (1)合并严重心、肝、肾等重要脏器功能障碍或严重精神、认知障碍，无法配合评估； (2)研究者判断不适宜参加者。 2.照护者排除标准： (1)无法按时随访或无稳定联系方式

Exclusion criteria：

1. Patient exclusion criteria: (1) Combined severe dysfunction of important organs such as the heart, liver, or kidneys, or severe mental or cognitive disorders, making it impossible to cooperate with the assessment; (2) Participants deemed unsuitable by the researcher. 2. Caregiver exclusion criteria: (1) Unable to follow up on time or without stable contact information
