利用4D flow CMR技术探索肥厚型心肌病(包括梗阻及非梗阻)患者左心室KE及血流组分参数变化特点,以ACC/AHA HCM 风险模型为参考,初步建立4D-flow技术在HCM患者中风险分层阈值,并对患者进行MACE事件随访,探索预测不良事件的血流动力学标志物。 |

Objectives of Study: |
Utilizing 4D flow CMR technology, this study investigates the characteristics of left ventricular kinetic energy (KE) and blood flow component parameters in patients with non-obstructive hypertrophic cardiomyopathy (HCM), including both obstructive and non-obstructive subtypes. Using the ACC/AHA HCM risk model as a reference, we aim to preliminarily establish risk stratification thresholds based on 4D-flow parameters in HCM patients. Follow-up for major adverse cardiac events (MACE) will be conducted to identify hemodynamic biomarkers predictive of adverse outcomes. |

纳入标准: |
1. 符合肥厚型心肌病诊断标准:心肌非对称性增厚,心肌厚度≥15 mm,增厚心肌厚度/左室中段下壁厚度>1.5;当存在HCM家族史或基因检测结果为阳性时,室壁厚度≥13 mm即可诊断。 |

Inclusion criteria |
1. The diagnostic criteria for HCM were: (1) asymmetric left ventricular hypertrophy with a maximal wall thickness ≥15 mm and a thickness ratio (affected segment/inferior wall) >1.5; or a wall thickness ≥13 mm in individuals with a positive family history or causative genetic mutation. |

排除标准: |
1. 其他原因引起的心肌肥厚(如浸润性心肌病,如淀粉样变性,法布雷病等); 2. 炎性心肌病; 3. 冠心病; 4. 未控制的高血压; 5. 主动脉瓣疾病; 6. HCM消融术后; 7. 对比剂过敏; 8. 幽闭恐惧症; 9. 图像质量不达标。 |

Exclusion criteria: |
1. Exclusion criteria included: Myocardial hypertrophy secondary to other causes (e.g., infiltrative cardiomyopathies such as amyloidosis or Fabry disease); 2. Inflammatory cardiomyopathy; 3. Coronary artery disease; 4. Uncontrolled hypertension; 5. Aortic valve disease; 6. Post-ablation therapy for HCM; 7. Allergy to contrast agents; 8. Claustrophobia; 9. Inadequate image quality. |
