研究目的：

通过锥形束计算机断层扫描（CBCT）测量，比较在种植同期上颌窦外提升术中，采用新型固位骨块回置法与传统游离骨块回置法，在术前、术后即刻与术后6个月时骨块的在位率、上颌窦区内牙槽骨高度、窦内骨增益和根尖骨高度等。同时评估两组患者术中及术后的不良事件与并发症发生情况，以科学评估新型骨块回置法的临床优势与安全性。  

Objectives of Study：

To compare, using cone-beam computed tomography (CBCT), the bone block survival rate, alveolar bone height within the maxillary sinus area, intrasinus bone gain, and apical bone height between the novel retention bone block repositioning method and the traditional free bone block repositioning method in maxillary sinus floor elevation with simultaneous implant placement. These parameters were assessed preoperatively, immediately postoperatively, and at six months post-surgery. Additionally, intraoperative and postoperative adverse events and complications were evaluated in both groups in order to scientifically assess the clinical advantages and safety of the novel bone block repositioning technique.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.2024 年 9 月至 2025 年 9 月期间与浙江大学医学院附属口腔医院接受27种植同期上颌窦外提升术的所有患者； 2. 年龄>=18 周岁； 3. 缺失一颗及以上上颌后牙，且愈合时间超过 3 个月； 4. 具备完整基础资料及详细手术记录； 5. 术前（T0）、术后即刻（T1）及术后 6 个月（T2）的 CBCT 影像资料齐全； 6. T0 期缺失牙部位 CBCT 显示的牙槽骨高度（RBH）<=6 毫米； 7. 术前无系统性疾病，且未接受过根尖周病或牙周病治疗。

Inclusion criteria

1. All patients who underwent 27 implants of concurrent maxillary sinus lift with the Affiliated Stomatological Hospital of Zhejiang University School of Medicine from September 2024 to September 2025; 2. Age>=18 years old; 3. Missing one or more maxillary posterior teeth with healing time of more than 3 months; 4. Have complete basic information and detailed surgical records; 5. Complete CBCT imaging data before surgery (T0), immediately after surgery (T1) and 6 months after surgery (T2); 6. Alveolar bone height (RBH) <=6 mm shown by CBCT at the missing tooth site in T0 stage; 7. No systemic disease before surgery, and no treatment for apical or periodontal disease.

排除标准：

1.上颌病变未接受过规范治疗；
2.具有可能影响正常骨代谢的情况，如患有严重影响骨代谢的全身性疾病（如未控制的糖尿病、骨质疏松症等）、有头颈部放疗史或正在使用/近期使用过（6个月内）影响骨代谢的药物（如双膦酸盐类、糖皮质激素等）；
3.处于妊娠期或哺乳期；
4.过去5年内接受过头颈部癌症放疗；
5.上颌窦存在急性或慢性炎症；
6.需要除上颌窦外提升术外，使用窦内移植材料进行额外的垂直或水平骨增量；
7.重度吸烟者（手术时每日吸烟量超过20支）；

Exclusion criteria：

1.Untreated maxillary pathologies that had not received standard treatment.
2.Presence of conditions that may affect normal bone metabolism, such as systemic diseases known to severely impact bone metabolism (e.g., uncontrolled diabetes mellitus, osteoporosis), a history of head and neck radiotherapy, or current/recent use (within the past 6 months) of medications affecting bone metabolism (e.g., bisphosphonates, corticosteroids).
3.Pregnancy or lactation.
4.History of head and neck cancer radiotherapy within the past 5 years.
5.Presence of acute or chronic inflammation in the maxillary sinus.
6.Requirement for additional vertical or horizontal bone augmentation using intrasinus graft materials beyond the maxillary sinus floor elevation via lateral window approach.
7.Heavy smokers (daily consumption exceeding 20 cigarettes at the time of surgery).
