研究目的：

探索双镜联合手术治疗早期胃癌的安全性和可行性，通过LECS+LSNNS的联合术式能够在保证肿瘤根治效果的前提下，实现对早期胃癌患者的胃功能保留，使患者营养状态和生活质量有显著获益。 建立标准化双镜联合手术（LECS+LSNNS）技术体系，完成5-10例早期胃癌手术（T1a/T1b期）  

Objectives of Study：

Explore the safety and feasibility of combined double-endoscopic surgery for early gastric cancer. The combined procedure of LECS+LSNNS can preserve gastric function in patients with early gastric cancer while ensuring radical tumor resection, significantly benefiting patients' nutritional status and quality of life. Establish a standardized technical system for combined laparoscopic endoscopic surgery (LECS+LSNNS), and complete 5-10 cases of early gastric cancer surgery (T1a/T1b stage)

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.年龄 18-65 岁； 2.术前经胃镜活检病理确诊为早期胃癌； 3.经 MDT 协作评估：病理科通过免疫组化明确肿瘤分化情况，内镜科通过超声内镜判定肿瘤分期及浸润深度，联合胃外科、消化内科、麻醉科评估符合 LECS+ICG-SNNS 适应症； 4.术前未接受化疗、放疗等抗肿瘤治疗； 5.患者及家属知情同意并签署同意书； 6.无手术、麻醉及 ICG 使用禁忌症，身体状况可耐受手术； 7.能够配合完成术后 12 个月随访及相关复查项目。

Inclusion criteria

1. Age: 18 - 65 years old; 2. Preoperative diagnosis of early gastric cancer confirmed by gastroscopy biopsy pathology; 3. Through MDT collaboration assessment: The pathology department determined the tumor differentiation through immunohistochemistry, the endoscopy department determined the tumor stage and invasion depth through endoscopic ultrasound, and the gastro surgery department, digestive internal medicine department, and anesthesia department jointly evaluated whether it met the indications of LECS + ICG-SNNS; 4. No anti-tumor treatment such as chemotherapy or radiotherapy was received before the surgery; 5. The patient and his/her family gave informed consent and signed the consent form; 6. There were no contraindications for surgery, anesthesia, or ICG use, and the physical condition was tolerable for the surgery; 7. The patient and his/her family could cooperate to complete the 12-month postoperative follow-up and related re-examination items.

排除标准：

1.经 MDT 协作评估不符合 LECS+ICG-SNNS 适应症者； 2.年龄＜18 岁或＞65 岁； 3.合并严重基础疾病：心功能＞II 级（NYHA）、重度高血压（收缩压>=180mmHg 或舒张压>=110mmHg）、严重肝肾功能不全（Child-Pugh>=10 分、肌酐清除率＜25ml/min）、呼吸功能不全、凝血功能障碍； 4.对吲哚菁绿（ICG）、麻醉药、头孢类抗生素等围手术期常用药物过敏； 5.合并完全性胃肠梗阻、急诊手术或姑息性手术者； 6.手术需联合切除脾、胰腺等其他脏器者； 7.恶液质或严重营养不良（白蛋白<=0g/L、BMI＜18）； 8.妊娠、哺乳期妇女或试验后 1 个月内有妊娠计划者（含男性受试者）； 9.近 3 个月参加其他临床试验者； 10.直接参与试验的申办者、研究者或其家庭成员； 11.病理证实为中晚期胃癌或其他类型肿瘤（鳞癌、神经内分泌肿瘤、胃肠间质瘤等）。

Exclusion criteria：

1. Those who do not meet the indications of LECS+ICG-SNNS after MDT collaborative assessment; 2. Age less than 18 years or more than 65 years; 3. Complicated with severe underlying diseases: cardiac function > grade II (NYHA), severe hypertension (systolic blood pressure >= 180 mmHg or diastolic blood pressure >= 110 mmHg), severe liver and kidney dysfunction (Child-Pugh >= 10 points, creatinine clearance rate < 25 ml/min), respiratory insufficiency, coagulation dysfunction; 4. Allergic to indocyanine green (ICG), anesthetics, cephalosporin antibiotics and other commonly used perioperative drugs; 5. Complicated with complete gastrointestinal obstruction, emergency surgery or palliative surgery; 6. Surgery requires combined resection of other organs such as spleen and pancreas; 7. Malnutrition or severe malnutrition (albumin <= 0 g/L, BMI < 18); 8. Pregnant, lactating women or those with pregnancy plans within 1 month after the trial (including male participants); 9. Those who have participated in other clinical trials in the past 3 months; 10. The sponsors, investigators or their family members directly involved in the trial; 11. Pathologically confirmed as advanced gastric cancer or other types of tumors (squamous cell carcinoma, neuroendocrine tumor, gastrointestinal stromal tumor, etc.).
