研究目的:

本研究旨在真实临床环境下评估基于个体脑功能剖分技术(pBFS)引导的经颅磁刺激(优点疗法POINT Therapy)在改善帕金森病(PD)患者运动症状中的有效性,探索在“药物维持时间延长”方面具有显著优势的患者亚型特征,同时探索基于个体脑功能剖分技术引导的经颅磁刺激是否通过改善类淋巴系统功能,提高脑内病理蛋白清除效率,进而延长药物有效维持时间,并建立类淋巴功能指标对药物维持时间获益的早期预测价值。  

Objectives of Study:

This study aims to evaluate the effectiveness of transcranial magnetic stimulation (TMS) guided by the individual brain functional segmentation technique (pBFS) in improving the motor symptoms of Parkinson's disease (PD) patients in a real clinical setting. It also aims to explore the characteristics of patient subtypes that have significant advantages in "extended drug maintenance time", and to investigate whether TMS guided by the individual brain functional segmentation technique can increase the efficiency of clearing pathological proteins in the brain by improving the function of the lymphatic system, thereby prolonging the effective maintenance time of drugs, and establish the early predictive value of lymphatic function indicators for the benefits of drug maintenance time.

Description for medicine or protocol of treatment in detail:

 

纳入标准:

1年龄不超过<=75岁; 2.符合2015年国际运动障碍学会诊断标准,诊断为帕金森病; 3.1<=Hoehn-Yahr分期<=3(药物“关期”时); 4.接受稳定的抗PD药物治疗至少1个月; 5.能够理解研究内容并签署知情同意书。

Inclusion criteria

1. Age should be no more than 75 years old; 2. Meet the diagnostic criteria of the International Parkinson's Disease Society in 2015 and be diagnosed with Parkinson's disease; 3. Hoehn-Yahr staging is between 1 and 3 (during the "off" period of medication); 4. Have received stable anti-PD medication treatment for at least 1 month; 5. Can understand the research content and sign the informed consent form.

排除标准:

1.有卒中等其他脑部器质性疾病病史者; 2.持续头部震颤影响高精度光学导航追踪的PD患者; 3.明显认知障碍影响配合(MMSE<=20分,较RCT方案放宽以符合真实世界特征); 4.既往接受DBS手术者; 5.有MRI或TMS治疗禁忌症(如幽闭恐惧症、心脏起搏器、体内金属植入物、癫痫发作史等); 6.孕妇及妊娠期妇女。

Exclusion criteria:

1. Those with a history of stroke or other brain organic diseases; 2. PD patients with persistent head tremors that affect the accuracy of optical navigation tracking; 3. Those with significant cognitive impairment affecting cooperation (MMSE <= 20 points, relaxed to meet the characteristics of the real world compared to the RCT protocol); 4. Those who have previously undergone DBS surgery; 5. Those with contraindications to MRI or TMS treatment (such as claustrophobia, cardiac pacemaker, internal metal implants, history of epilepsy attacks, etc.); 6. Pregnant women and women in the pregnancy period.
