研究目的：

建立关节镜辅助下关节盘复位术联合正畸正颌序列治疗下颌后缩畸形伴颞下颌关节骨关节病的治疗流程，并评价其临床有效性、稳定性。  

Objectives of Study：

To establish a standardized treatment protocol for arthroscopy-assisted disc repositioning combined with orthodontic-orthognathic sequential therapy in patients with mandibular retrognathia complicated by temporomandibular joint osteoarthritis, and to evaluate its clinical efficacy and stability.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.年龄大于18岁；
2.SNB小于75°，临床诊断为下颌后缩畸形，且伴有不可复性盘前移位及骨关节病；
3.主诉关节疼痛（VAS≥2）或张口受限（开口度≤35mm）持续6个月以上，保守治疗效果不显著；
4.同意进行关节镜辅助下关节盘复位术联合正畸正颌序列治疗，且术前和术后能配合随访收集临床资料、关节MRI、CBCT和头颅正侧位片；

Inclusion criteria

1.Age > 18 years; 2.SNB < 75°, clinically diagnosed with mandibular retrognathia, accompanied by anterior disc displacement without reduction (ADDwoR) and osteoarthrosis; 3.Chief complaint of joint pain (VAS >=2) or limited mouth opening (maximum interincisal opening <= 35 mm) persisting for more than 6 months, with unsatisfactory outcomes following conservative treatment; 4.Consent to undergo arthroscopically assisted disc repositioning combined with orthodontic-orthognathic sequential therapy, and willingness to cooperate with preoperative and postoperative follow-up for collection of clinical data, joint MRI, CBCT, and cephalometric radiographs (posteroanterior and lateral cephalograms);

排除标准：

1.涉及牙颌面的外伤、肿物、良性肥大、综合征引起的下颌后缩畸形；
2.关节盘变形严重难以关节镜下复位；
3.存在非TMD口面部疼痛障碍；
4.有关节手术史；
5.存在风湿性关节病等全身免疫相关疾病；
6.存在焦虑症、抑郁症等心理问题；

Exclusion criteria：

1.Mandibular retrognathia caused by maxillofacial trauma, tumors, benign hypertrophy, or syndromes;
2.Severe disc deformation rendering arthroscopic repositioning unfeasible;
3.Presence of non-TMD orofacial pain disorders;
4.History of previous joint surgery;
5.Presence of systemic immune-related diseases such as rheumatoid arthritis;
6.Presence of psychological disorders such as anxiety or depression;
