研究目的：

本研究拟通过拔牙导板的设计、3D打印和应用，在术前以3D打印精准地反映患牙及相邻组织，再根据模型设计出手术导板。术中先通过导板精准去骨，使要拔除的患牙显露，再依靠导板的指引去除牙冠，解除阻力，同时保护临近组织不受损伤，最终拔除患牙。  

Objectives of Study：

This study aims to design, 3D printing and make customized fixation plates, with help of the precise reflection of the impacted tooth and its adjacent tissues by 3D printing before operation. During the operation, the bone is precisely removed through the guidance of the fixation plate to expose the impated tooth. Then, the crown is removed under the guidance of the plate to eliminate resistance and protect the adjacent tissues before the impacted tooth is extracted.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.下颌完全骨埋伏阻生牙。 2.患牙有临床症状或正畸治疗需要拔除。 3.CBCT检查为水平及倒置阻生的患牙，且通过CBCT测量患牙牙冠最低点至下颌第二磨牙??平面距离大于15mm。 4.患侧下颌第二磨牙及第一磨牙无缺失。 5.否认全身性疾病。

Inclusion criteria

1.Completely impacted mandibular tooth. 2.The impacted tooth has clinical symptoms or needs to be extracted for orthodontic treatment. 3.CBCT shows that the tooth is horizontally or invertedly impacted, and the distance from the lowest point of the crown of the impacted tooth to the occlusal plane of the mandibular second molar is bigger than 15mm. 4.The mandibular first molar and second molar on the impacted side are intact. 5.Denial of systemic diseases.

排除标准：

1.张口受限（上下切牙切缘小于2.5cm）的患者。 2.CBCT示有颌骨囊肿等病变的患者。 3.对树脂等材料过敏的患者。

Exclusion criteria：

1.Limited mouth opening (the distance of the edges of the upper and lower incisors is less than 2.5mm). 2.CBCT shows the patient has jaw cysts or other lesions. 3.Patients allegic to materials such as resin.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

原始数据使用院内电子病历系统记录，该系统为内部系统，仅供院内访问。病例报告表及项目专用电子表格进行数据采集时，由专人进行，以患者编号代替患者姓名，不记录敏感信息，治疗信息的采集符合最小必要性原则。充分保障患者的隐/私权利及数据的真实性、科学性、准确性。

The raw data were recorded using the hospital electronic medical record system, which is an internal system and accessible only to the hospital. Data collection from the case report form and the project-specific spreadsheet should be carried out by specially-assigned person, patient number should be used instead of patient name, sensitive information should not be recorded, and collection of treatment information should comply with the principle of least necessity. The privacy rights of patients and the authenticity, scientificity and accuracy of data are fully guaranteed.
