研究目的: |
1.建立一套基于SCAN靶点的、融合无创定位与闭环神经调控的个体化精准治疗体系,系统验证皮层电刺激-深部脑刺激联合治疗帕金森病冻结步态的临床有效性与安全性,明确该疗法在改善冻结步态发作频率、步态启动及稳定性等方面的总体疗效。 2.阐明基于SCNA环路定位的皮层电刺激-深部脑刺激联合治疗调控皮层-基底节-丘脑运动环路的动态神经机制,揭示其对冻结步态相关神经振荡异常及功能连接紊乱的干预作用。 3.优化神经生物标志物的筛选与解码算法,精准解码患者运动意图,识别帕金森病相关的特定脑网络功能异常,将解码信息实时传输至外骨骼,通过外骨骼驱动步态,形成“外周辅助”与“中枢干预”的协同治疗,促进大脑神经通路的功能重塑,从根本上改善运动控制能力。 |

Objectives of Study: |
1. Establish an individualized precision treatment system based on SCAN targets, integrating non-invasive positioning and closed-loop neural regulation. Systematically verify the clinical effectiveness and safety of the combined treatment of cortical electrical stimulation and deep brain stimulation for Parkinson's disease-related freezing gait, and clearly define the overall therapeutic efficacy of this therapy in improving the frequency of freezing gait episodes, gait initiation, and stability. 2. Clarify the dynamic neural mechanism of cortical electrical stimulation-deep brain stimulation combined treatment based on SCNA circuit positioning, and reveal its intervention effect on abnormal neural oscillations and functional connectivity disorders related to freezing gait. 3. Optimize the screening and decoding algorithms of neurobiological markers, precisely decode patients' movement intentions, identify specific brain network functional abnormalities related to Parkinson's disease, and transmit the decoding information to the exoskeleton. Through the exoskeleton, drive gait and form a collaborative treatment of "peripheral assistance" and "central intervention", promoting the functional remodeling of brain neural pathways and fundamentally improving motor control ability. |

纳入标准: |
1.符合国际运动障碍协会帕金森病临床诊断标准; 2.存在明确的致残性的冻结步态(New Freezing of Gait Questionnaire总分≥ 10); 3.药物抵抗型冻结步态; 4.Hoehn-Yahr分期在III-IV期(开期); 6.年龄18-80岁; 7.蒙特利尔认知评估量表评分≥ 24,排除痴呆; 8.能够理解研究内容并签署知情同意书; 9.无穿戴式传感器及外骨骼设备佩戴禁忌症(如下肢皮肤破溃、严重踝关节畸形等),可配合完成步态数据采集。 |

Inclusion criteria |
1. Meets the clinical diagnostic criteria of Parkinson's disease by the International Parkinson's Disease Association; 2. Has a definite disabling freezing gait (total score of the New Freezing of Gait Questionnaire ≥ 10); 3. Drug-resistant freezing gait; 4. Hoehn-Yahr stage is in III-IV (open phase); 6. Age between 18 and 80 years old; 7. Montreal Cognitive Assessment Scale score ≥ 24, excluding dementia; 8. Can understand the research content and sign the informed consent form; 9. Has no contraindications for wearing wearable sensors and exoskeleton devices (such as skin ulceration of the lower limbs, severe ankle deformity, etc.), and can cooperate to complete gait data collection. |

1. Secondary Parkinsonism or Parkinsonian superimposed syndrome; 2. Severe internal medical conditions or mental disorders; 3. History of epilepsy, intracranial metal implants, cardiac pacemakers; 4. Presence of MRI or TMS contraindications; 5. Previous DBS surgery and contraindications for DBS or MCS implantation; 6. Suffering from other neurological or skeletal-muscular system diseases that may affect gait; |
