本课题拟利用川渝地区多家市级医院的历年住院电子病历数据,结合国家气象站和环保监测站以及卫星遥感反演的大气污染/气象数据,定量分析不同污染和气象暴露水平与循环系统疾病住院人次之间的关联强度与规律。重点关注以下科研问题: ? 滞后效应:评估空气污染物和气象因素对心血管事件的急性效应是否存在滞后影响(lag效应),以及滞后效应的持续时间和非线性暴露-反应关系(例如考虑lag0~7天、lag0~14天的累积效应)。 ? 季节性差异:比较污染物和气象因素在不同季节对循环系统疾病住院的影响是否存在显著差异,探讨冬季和夏季等极端气候时期的效应强度变化。 ? 人群易感性:通过人群分层分析,考察老年人、不同性别、以及有基础疾病(如高血压、糖尿病等)患者是否对环境暴露更为敏感,从而确定易感亚人群。 ? 空间异质性:结合多中心数据,比较不同城市、区域之间空气污染暴露水平与心血管事件风险关系的差异,探讨地理和社会经济因素对污染健康效应的调节作用。 |

Objectives of Study: |
This project aims to utilize historical inpatient electronic medical record (EMR) data from multiple municipal hospitals in the Sichuan–Chongqing region, integrated with data from national meteorological stations, environmental monitoring stations, and satellite remote-sensing–derived air pollution and meteorological datasets. The objective is to quantitatively analyze the strength and patterns of associations between different levels of air pollution and meteorological exposures and hospital admissions for circulatory system diseases. The study will focus on the following key scientific questions: ? Lag effects: To evaluate whether air pollutants and meteorological factors exert delayed (lag) acute effects on cardiovascular events, and to characterize the duration of these lag effects and potential nonlinear exposure–response relationships (e.g., cumulative effects over lag 0–7 days and lag 0–14 days). ? Seasonal differences: To compare whether the effects of pollutants and meteorological factors on hospital admissions for circulatory diseases differ significantly across seasons, with particular attention to variations in effect magnitude during extreme climate periods such as winter and summer. ? Population susceptibility: Through stratified analyses, to investigate whether older adults, different sexes, and patients with underlying diseases (e.g., hypertension, diabetes) are more sensitive to environmental exposures, thereby identifying vulnerable subpopulations. ? Spatial heterogeneity: By integrating multicenter data, to compare differences in the relationships between air pollution exposure levels and cardiovascular event risk across different cities and regions, and to explore the modifying roles of geographic and socioeconomic factors on pollution-related health effects. |

本研究的数据采集和管理由病例记录表(Case Record Form, CRF)和电子数据采集与管理系统(Electronic Data Capture, EDC)两部分组成。 各参与中心依据统一的数据提取标准操作流程(SOP),从本单位住院电子病历系统中提取研究所需信息,并填写标准化的电子病例记录表(CRF)。CRF内容包括人口学信息、诊断信息、住院情况及结局指标等。 所有CRF数据在各参与中心完成脱敏处理后,通过加密的电子文件方式安全传输至牵头单位,由牵头单位建立集中式电子数据管理系统(基于文件的EDC),用于数据存储、整理、核查、版本管理和质量控制。 牵头单位负责统一的数据管理和统计分析,各参与中心仅可访问本中心数据或经授权的汇总数据,以确保数据安全和研究质量。 |

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture: |
Data collection and management consist of Case Record Forms (CRFs) and an electronic data capture (EDC) system. Standardized electronic CRFs are used at each participating center to extract required information from hospital electronic medical records. After de-identification, CRF files are securely transferred to the leading center, where a centralized file-based EDC system is established for data storage, quality control, and analysis. |
