研究目的: |
慢性肾脏病(chronic kidney disease,CKD)是指各种原因引起的肾脏结构或功能异常,伴或不伴肾小球滤过率(glomerular flitration rate, GFR)下降;或不明原因的GFR下降≥3个月。认知功能障碍是CKD较常见的合并症之一, 然而,CKD相关认知功能障碍的发病机制十分复杂且尚未完全明确,目前临床工作亦缺乏有效的治疗方法。继续深入探讨 CKD相关认知功能障碍发病的机制对预防和治疗认知功能障碍、改善CKD患者的认知状态具有十分重要的意义。在研究CKD诱导认知功能障碍发生的相关研究中,有研究提出肠道菌群及其代谢产物参与了CKD诱导的认知功能障。氧化三甲胺(trimethylamine N-Oxide,TMAO)是肠道菌群代谢胆碱等物质产生的代谢产物。在CKD患者中,TMAO是明显增加的,除了肾功能下降TMAO不能排除的原因外,CKD导致肠道菌群紊乱,肠道菌群有害代谢物增加也是其原因。TMAO可以穿过血脑屏障到达中枢神经。有研究认为,高循环浓度的TMAO可能通过增加脑核因子κB(nuclear factor kappa-B,NF-κB)和促炎细胞因子释放促进神经炎症并降低认知功。同时,TMAO还可激活星形胶质细胞和小胶质细胞,在外周和大脑中触发级联的炎症反应。因此,我们计划对慢性肾脏病以及无慢性肾脏病的健康受试者用蒙特利尔认知评估量表进行认知功能评估,并抽取受试者3ml血液,分离血清用ELISA检测受试者血清中TMAO的含量,分析TMAO与认知功能之间的相关性。 |

Objectives of Study: |
CKD is defined as abnormalities in kidney structure or function. Cognitive dysfunction represents one of the most prevalent comorbidities associated with CKD. However, the underlying pathogenesis of CKD-related cognitive impairment remains highly complex and incompletely elucidated. Among studies exploring factors contributing to CKD-induced cognitive dysfunction, emerging evidence implicates the gut microbiota and its metabolites as key mediators. TMAO, a gut microbial metabolite derived from dietary precursors such as choline, is markedly elevated in CKD patients.Notably, TMAO is capable of crossing the BBB and accumulating in the CNS. Preclinical and clinical studies suggest that elevated systemic TMAO levels may exacerbate neuroinflammation and impair cognitive function, potentially via activation of the NF-κB signaling pathway and subsequent upregulation of pro-inflammatory cytokines. Moreover, TMAO has been shown to activate astrocytes and microglia, thereby triggering inflammatory cascades both peripherally and within the brain parenchyma. Therefore, we plan to administer the MoCA to evaluate cognitive function in both patients with CKD and healthy controls without CKD. In addition, each participant will have 3 milliliters of venous blood sample drawn; the serum will be separated by centrifugation to measure the TMAO concentration. Finally, we will analyze the correlation between the serum TMAO level and cognitive performance. |

Inclusion criteria |
1. Age between 20 and 65 years of both sexes. 2. Hospitalized patients with established chronic kidney disease. Patients who consented for blood sampling and cognitive status assessment. 4. Healthy control group: people aged 20 to 65 years without chronic kidney disease who came to our hospital for physical examination. |
